    Abstract
    Objective: To evaluate the relationship between the Systemic Immune-Inflammation Index (SII) and Systemic Inflammation Response Index (SIRI) with Major Adverse Cardiovascular Events (MACE) in patients with acute myocardial infarction (AMI) at Adam Malik Hospital, Medan. Methods: This cross-sectional study included 224 AMI patients from January–December 2023. Hematological parameters (neutrophil, lymphocyte, monocyte, platelet counts) and troponin I were measured, and SII and SIRI were calculated. MACE was recorded during hospitalization. Data were analyzed using Mann–Whitney U and logistic regression tests (p < 0.05). Results: Most subjects were male (79.5%) with a mean age of 56.49 ± 10.39 years. STEMI was diagnosed in 63.8% of patients. MACE occurred in 57 patients (25.4%), with mortality being most frequent (6.7%). Mean SII and SIRI were 1,090.52 (214.04–9,298.8) and 3.83 (0.35–31.19), respectively. Both SII and SIRI were significantly associated with MACE (p = 0.002 and p = 0.001), with SIRI showing predictive value (OR 1.233; 95% CI 1.151–1.320). Conclusion: SII and SIRI are significantly associated with in-hospital MACE in AMI patients, but only SIRI demonstrates predictive value. Keywords: Acute Myocardia l Infarction, SII, SIRI, MACE event
