Background: Chronic kidney disease (CKD) is characterized by progressive kidney damage that leads to anemia, an increase in Red Cell Distribution Width (RDW) and impaired iron status, such as increased ferritin and decreased Total Iron Binding Capacity (TIBC). Although RDW is useful in differentiating anemia types, its relationship to iron profile levels remains debated. Objective: To determine the relationship between RDW and TIBC results and ferritin levels in chronic kidney disease patients undergoing hemodialysis at Adam Malik Hospital. Methods: The study was a retrospective study involving 114 CKD patients undergoing hemodialysis at Adam Malik Hospital in 2023. Patients involved in this study were patients aged ≥ 18 years, undergoing outpatient hemodialysis, and had complete medical record. The collected data were then analyzed using the Spearman correlation test. Results: The majority of patients in this study were male (62.3%), aged 46-60 years (43.0%), duration of hemodialysis <5 years (70.2%), and had diabetes (56.1%). Based on laboratory characteristics, the majority of patients had normal RDW levels (60.5%), increased ferritin (78.1%), and decreased TIBC (69.3%). The results of the correlation analysis did not show a significant relationship between RDW and TIBC (r = -0.125; p = 0.185) and RDW and ferritin (r = 0.151; p = 0.110). Hemodialysis duration was not significantly associated with hemoglobin levels or RDW values, with p values of 0.956 and p values of 0.463, respectively. Conclusion: There is no relationship between RDW to TIBC and ferritin.en_US
