Analysis of Maintenance Failures on Palm Oil Processing Screw Press Machines Using Failure Mode and Effect Analysis (FMEA)
Abstract
PT Tolan Tiga Indonesia Perlabian Palm Oil Mill is a palm oil processing plant
that produces final products such as crude palm oil (CPO) and kernel. The
Company has already implanted preventive maintenance, corrective maintenance,
and predictive maintenance methods. However, unexpected breakdowns often
occurs due to several possible factors. The machine that experienced the most
downtime was the screw press machine, which had 65 instances from January to
July 2024. Based on the high frequency of breakdowns in the screw press machine,
a maintenance failure analysis is necessary. The analysis method used is Failure
Mode and Effect Analysis (FMEA). Based on FMEA results, the Risk Priority
Number (RPN) calculation is performed to determine the critical components of the
machine. An analysis using Fault Tree Analysis (FTA) is conducted on the
components with the highest RPN. The FMEA results indicate that the most critical
part is the Intermediate Body and Cone Section. The components with the highest
RPN are Worm Screw with RPN of 224 and Press Cage with RPN of 210. The FTA
results indicate the need for increased supervision of operators, additional training
for operators and mechanics, scheduling for component realignment and
maintenance inspection on each component, stricter cleaning area regulation, and
evaluation of the specification, lifespan, and capacity of the machine and
components.
