dc.description.abstractObjective : To evaluate the association between Systemic Immune-Inflammation Index (SII) and Neutrophil-Lymphocyte Ratio (NLR) with Pneumonia Severity Index (PSI) scores in patients with bacterial pneumonia at Adam Malik Hospital in Medan. Methods: This cross-sectional observational study analyzed secondary data from medical records of bacterial pneumonia patients admitted between January and December 2023. All patients underwent complete blood count, blood/sputum cultures, and PSI scoring. SII was calculated as platelet × neutrophil/lymphocyte, and NLR as neutrophil/lymphocyte. Data were analyzed using Spearman correlation with significance set at p < 0.05. Results: A total of 76 patients were included, predominantly male (67.1%) with a mean age of 57.78 ± 15.74 years. The most common comorbidity was kidney disease (11.8%). Gram-negative bacteria accounted for 57.9% of isolates, with Acinetobacter baumannii as the most frequent pathogen (15.8%). The platelet count was 216,404 ± 107,078.22, leukocyte count 15,819.49 ± 11,284.4, NLR was 12.01, SII was 2,385.79, and mean PSI score was calculated as 53.55 ± 27.79. Correlation analysis showed no significant association between NLR (r = 0.199; p = 0.086) or SII (r = 0.161; p = 0.165) and PSI scores. Conclusion: Neither NLR nor SII showed a significant correlation with PSI scores in bacterial pneumonia. Larger prospective studies are warranted to further evaluate their potential role as simple hematological biomarkers for predicting disease severity.en_US
