Introduction: Iron deficiency anemia is one of the most common hematological disorders found in pregnant women and remains a major public health challenge in Indonesia. Preventive measures have been implemented through the provision of iron and folic acid supplementation (tablet tambah darah/TTD); however, the level of adherence among pregnant women in consuming these supplements remains relatively low. One of the factors suspected to influence this issue is the level of understanding pregnant women have regarding the benefits and importance of iron supplementation during pregnancy. Objective: To determine the relationship between the level of knowledge of pregnant women about iron deficiency anemia during pregnancy and their adherence to iron tablet consumption at Kedai Durian Public Health Center, Medan Johor District. Method: This study used an analytical cross-sectional design involving 35 pregnant women in their first to third trimesters who met the inclusion criteria. Data were collected using a knowledge questionnaire and the Morisky Medication Adherence Scale (MMAS-8). Statistical analysis was performed using the Chi-Square test. Results: The Chi-Square test showed no significant relationship between the level of knowledge and adherence to iron tablet consumption (p = 0.328). However, there was a significant relationship between educational level and knowledge (p < 0.001). Conclusion. There is no significant relationship between pregnant women’s knowledge about iron deficiency anemia and adherence to iron tablet consumption; however, educational level plays an important role in influencing maternal knowledge. In addition, gravida is also found to play a significant role in determining adherence to iron tablet consumption.en_US
