Many disorders and diseases can affect the human digestive system, one of which is ascites. Ascites is a condition where abnormal fluid accumulates in the abdominal cavity, causing the abdomen to enlarge. The primary cause of ascites is liver damage. When ascites reaches a severe level, a clinical procedure involving the insertion of a special needle is required to drain the fluid, a procedure known as paracentesis. Currently, learning media for the paracentesis ascites procedure includes the use of mannequins, video recordings accompanied by theoretical learning, and live demonstrations by lecturers. However, these learning methods are considered non-immersive and lack interactivity. Therefore, this study designs a virtual reality application using Unreal Engine to simulate ascites paracentesis as an alternative learning medium. The testing was conducted on 10 medical students, who were then asked to express their opinions through a questionnaire using a 1 to 5 scale to measure user perception of the application. Based on the questionnaire results, the information relevance received a rating of 89%, the application system received a rating of 91%, and the overall user experience during the VR simulation received a rating of 90%.
