Background: Chronic maxillary rhinosinusitis (CMRS) is an inflammatory condition of the sinus mucosa lasting more than 12 weeks and is frequently associated with anatomical variations in the osteomeatal complex. Structures such as the ethmoid infundibulum (EI), uncinate process (UP), and nasal septal deviation (NSD) are thought to influence the development of CMRS. Computed tomography of the paranasal sinuses (CT PNS) is the imaging modality of choice to evaluate these anatomical structures in detail. Objective: To evaluate the association between the length and width of the ethmoid infundibulum, types of uncinate process, and nasal septal deviation with the incidence of CMRS based on CT PNS findings at RS CPL Medan. Methods: This was an analytical study with a cross-sectional design involving patients undergoing CT PNS at RS CPL Medan. Data included measurements of EI, classification of UP, and angle of septal deviation. Results: A total of 100 maxilary sinuses were included. The mean EI length in patients with CMRS was 5.9 ± 2.7 mm, significantly longer than in those without CMRS (2.8 ± 1.1 mm), p < 0.001. EI width was also significantly associated (p < 0.05). The most common UP type among CMRS patients was type A (47%), while types B and C predominated in the non-CMRS group (p < 0.001). Septal deviation ≥10° was more frequent in CMRS patients (31%) than in non-CMRS patients (13%), p < 0.05. All anatomical parameters showed significant associations with CMRS occurrence. Conclusion: There is a significant association between the dimensions of the ethmoid infundibulum, uncinate process type, and nasal septal deviation with the incidence of chronic maxillary rhinosinusitis based on CT PNS findings at RS CPL Medan.en_US
