Introduction: Hearing loss is a common occupational health problem among military personnel due to repeated exposure to high-intensity noise. Pure tone audiometry is the gold standard for diagnosis but is often limited by resource availability. Therefore, a simple and accurate screening tool such as the Hearing Handicap Inventory for Adults–Screening version (HHIA-S) is required. Objective: To evaluate the diagnostic accuracy of the HHIA-S questionnaire as a screening tool for hearing loss among Indonesian National Armed Forces personnel. Methods: This cross-sectional analytic study involved 44 enlisted soldiers at Yonif 100/Prajurit Setia, Binjai. All participants underwent pure tone audiometry and completed the HHIA-S questionnaire. Diagnostic accuracy was evaluated using ROC curve analysis. Results: Hearing loss was identified in 79.5% of participants, all of which were sensorineural. The AUC was 0.953 (p < 0.001). The optimal cut-off score was ≥10, with sensitivity 86.1%, specificity 87.5%, and accuracy 86.4%. Conclusion: HHIA-S demonstrates excellent accuracy as an initial screening tool for hearing loss among military personnel.
