Analysis of Unsafe Action Reports at PT. Tirta Lyonnaise Medan for the Period 2020 – 2024
Abstract
Unsafe acts can be caused by human error. An Unsafe Action Report is a document used to record unsafe acts in the workplace. The purpose of this study is to analyze the Unsafe Action Report at PT. Tirta Lyonnaise Medan for the period 2020-2024. Data analysis in this study was conducted univariately and presented in the form of frequency distribution tables, percentages, and descriptive narratives. From the research results that have been described, it is clear that PT. Tirta Lyonnaise Medan still faces significant challenges related to unsafe work behavior (unsafe actions). In the period 2020 to 2024, a total of eight hazardous actions were identified, with an alarming spike in 2024, which contributed 50 percent of the total incidents. The two most dominant types of hazardous actions were using damaged tools and working with incorrect body postures, indicating the need for emphasis on ergonomic training and pre-employment inspections. On the positive side, there are indications of awareness and active participation from workers, where each hazardous action report is followed by suggestions for improvement by the reporter, demonstrating a culture of care that needs to be continuously improved. The response from the Health, Safety, and Environment (HSE) team was also relatively fast and firm, by immediately giving warnings and stopping potentially dangerous work, which is the right practice in incident management. In carrying out risk control at PT. Tirta Lyonnaise Medan uses a hierarchy of control, namely elimination, substitution, engineering, administrative and personal protective equipment. It is expected that PT. Tirta Lyonnaise Medan will increase supervision in the field, to ensure compliance with work procedures and ensure workers comply with work safety regulations and work according to work procedures implemented in the company.
