dc.description.abstractINTRODUCTION The anterior cruciate ligament (ACL) is one of the primary stabilizers of the knee and is among the most frequently injured structures, especially in sports involving pivoting or sudden changes in direction. ACL injuries significantly impact daily activities and increase the risk of osteoarthritis in the long term. Radiological assessment using simple measurements, such as the intercondylar roof inclination angle (IRIA) on lateral knee radiographs, has been suggested as a predictor of ACL injury. However, previous studies have reported inconsistent results, and data from Indonesia remain limited. Therefore, this study aimed to analyze the difference in IRIA values between normal individuals and patients with ACL injury. METHODS This was an observational analytic study with a case-control design. A total of 54 subjects were included, consisting of 26 patients with ACL injury and 28 controls without ACL injury. IRIA was measured on true lateral knee radiographs using Blumensaat line method. Statistical analysis was performed using an independent t-test, with significance set at p<0.05. RESULTS Of the 54 subjects, 39 were male and 15 female, with a mean age of 26.56±6.84 years. ACL injury group: mean IRIA 35.63˚±4.43˚. Control group: mean IRIA 32.36˚±2.82˚.Overall mean IRIA: 33.94˚±4.00˚. Independent t-test analysis revealed a significant difference in IRIA values between the ACL injury group and controls (p=0.001). CONCLUSION There is a significant difference in intercondylar roof inclination angle between individuals with ACL injury and those without. A higher IRIA value is associated with an increased risk of ACL injury. Measurement of IRIA provides a simple and rapid screening tool to identify individuals at risk of ACL injury, particularly those actively engaged in sports.en_US
