Introduction. Colorectal cancer has a high incidence and mortality rate, with nutritional status playing a significant role in prognosis, therapeutic response, and quality of life. Assessment through BMI and PNI reflects the patient's nutrition and immunity. Objective. To determine the description of nutritional status based on BMI and PNI in colorectal cancer patients at Haji Adam Malik General Hospital, Medan in 2022-2025. Methods. This study is a descriptive study with a cross-sectional approach using secondary data from the medical records of colorectal cancer patients treated at Haji Adam Malik General Hospital, Medan in 2022-2025. The study sample consisted of 91 patients who met the inclusion and exclusion criteria. The variables studied included gender, age, weight, height, BMI, albumin levels, total lymphocyte count, and PNI values. Data were analyzed descriptively and presented in the form of frequency distributions and percentages. Results and Discussion. Most colorectal cancer patients were male (52.7%), with the largest age group being 51-60 years (34.1%). Based on BMI, the majority of patients had normal nutritional status (63.7%), followed by underweight (13.2%) and overweight (13.2%). Most patients had normal albumin levels (56%), normal lymphocyte counts (71.4%), but PNI values <46.6 were found in 96.7% of patients, indicating that most patients had poor nutritional and immune status. Conclusion: The majority of colorectal cancer patients had normal BMI but low PNI, therefore, nutritional assessment using BMI and PNI is important for monitoring patient condition and prognosis.en_US
