    Abstract
    Introduction: Blood glucose levels have been recognized as a key factor influencing outcomes in patients with Acute Coronary Syndrome (ACS). This study aims to investigate the association between admission blood glucose levels and the occurrence of major adverse cardiovascular events (MACE) in ACS patients. Methods This retrospective observational study analyzed secondary data from medical records of patients with ACS admitted to Adam Malik Hospital between 2022 and 2023. Data on admission blood glucose levels, age, diabetes mellitus, dyslipidemia, hypertension and MACE were collected. The chi-square test or Fisher's exact test was applied for bivariate analysis. Variables with p-value <0.25 in the bivariate analysis were included in a multivariate logistic regression model. Results: The study included 100 ACS patients, with an average age of 59 ± 10.1 years. MACE was found in 47 (47%) patients. The MACE rate was significantly higher in hyperglycemic patients (73.5%) than in normoglycemic patients (33.3%). A significant association was found between admission blood glucose levels and MACE (p=0.001). Logistic regression analysis revealed that admission blood glucose levels were the only variable significantly associated with MACE (OR 0.16; p=0.001). Conclusion: This study confirms that admission blood glucose levels are significantly associated with MACE in ACS patients. Monitoring blood glucose at admission should be part of routine assessments for early identification and management of hyperglycemia to reduce the risk of MACE
