Introduction: Diabetic retinopathy is a microvascular complication frequently encountered in individuals with diabetes mellitus. Its development is believed to be influenced by long-term glycemic exposure, which can be reflected by hemoglobin A1c (HbA1c) levels. Objective: This study was conducted to assess the relationship between HbA1c levels and the duration of diabetes mellitus with the severity of diabetic retinopathy among patients at Prof. dr. Chairuddin P. Lubis Universitas Sumatera Utara Hospital, Universitas Sumatera Utara, Medan, 20155, Indonesia. Method: An analytic cross-sectional design was applied, involving 29 patients with diabetes mellitus who met the inclusion criteria. Data were collected between September and October using a total sampling approach. Results and Discussion: The findings demonstrated a statistically significant association between HbA1c levels and the severity of diabetic retinopathy (p = 0.049). Conversely, no significant association was identified between the duration of diabetes mellitus and retinopathy severity (p = 0.286). Conclusion: Higher HbA1c levels are associated with more severe stages of diabetic retinopathy, whereas the duration of diabetes mellitus does not appear to be significantly related to retinopathy severity.en_US
