Background. Inflammation plays an essential role in the pathogenesis of insulin resistance, which is the central mechanism underlying Type 2 Diabetes Mellitus (T2DM). Simple and cost-effective inflammatory markers derived from complete blood count, such as the neutrophil-to-lymphocyte ratio (NLR) and the lymphocyte-to-monocyte ratio (LMR), are frequently used to predict complications associated with T2DM. Objective. This study aimed to evaluate and compare NLR and LMR values in T2DM patients grouped into obese and non-obese categories at Prof. dr. Chairuddin P. Lubis Universitas Sumatera Utara Hospital, Universitas Sumatera Utara, Medan, 20155, INDONESIA. Methods. A cross-sectional was conducted in this study. This study involved 46 T2DM and classified according to body mass index (BMI), comprising 23 obese and 23 non-obese individuals. Data were obtained from medical records and analyzed using SPSS version 25 for Windows. Results. The mean LMR was higher in non-obese patients compared with obese patients, this difference was not statistically significant (p = 0,475). In contrast, obese patients demonstrated a higher mean NLR, with the difference reaching statistical significance (p = 0.036). Conclusion. Obesity among T2DM patients is significantly associated with elevated NLR values, whereas LMR does not exhibit a meaningful association with obesity status. These findings indicate that NLR may serve as an additional indicator for monitoring inflammatory status in obese T2DM patients.en_US
