Background. Infarct volume is often considered related to stroke severity andcognitive impairment, but this relationship is not always consistent, particularly insmall infarcts. Objective. To evaluate the association between infarct volume and stroke severity assessed by NIHSS, as well as cognitive impairment assessed by MoCA-Ina, in patients with acute ischemic stroke. Methods. A cross-sectional study was conducted on patients with acute ischemic stroke at Adam Malik Hospital. Infarct volume was measured using non-contrast CT scans and categorized as <70,000 mm³ (Non-Large Infarct) and ≥70,000 mm³ (Large Infarct). Stroke severity was assessed using NIHSS, while cognitive function was evaluated using MoCA-Ina. Fisher’s Exact Test was used for statistical analysis. Results. Most patients had infarct volumes <70,000 mm³. No significant association was found between infarct volume and stroke severity (p = 0.155) or cognitive impairment (p = 0.435). These findings suggest that infarct size alone does not determine neurological deficits or cognitive outcome. Conclusion. Infarct volume is not significantly associated with stroke severity or cognitive impairment. Lesion location and other clinical factors may play a more important role in determining neurological and cognitive outcomesen_US
