    Abstract
Introduction: Intraokular pressure reduction is the primary target in the management of POAG and PACG. Non-pharmacological management using a treadmill test can reduce IOP. IOP is reduced through the dimensions of the Schlemm's canal. This study aimed to analyze the differences in IOP and OPP in POAG and PACG patients before and after the treadmill test. Methods: This study was an experimental study with a pretest-posttest control group design. The subjects were normal, POAG, and PACG patients at the Polyclinic of the Prof. Dr. Chairuddin P. Lubis Teaching Hospital, University of North Sumatra. This study used a treadmill test with light and moderate intensity for each group, with 20 participants per group. The results included demographic data, IOP, and OPP values. Results: The majority of the POAG and PACG groups were male (70%) and male (50%). The average age was 50.2-50.4 years. The lowest IOP values in the light intensity group were in the control group (11.25±0.57 mmHg), followed by PACG (17.58±0.95 mmHg) and POAG (17.82 ± 0.60 mmHg), with an OPP of 40.25 mmHg in POAG and 41.79±1.62 mmHg in PACG. The IOP values in the moderate intensity group were most pronounced in POAG, from 19.67±1.58 to 17.17±1.02 mmHg, with the highest OPP in the POAG group (45.57±2.14 mmHg). Conclusion: The results of changes in IOP values after a treadmill test at a light intensity showed a decrease in the normal and PACG groups, while the treadmill test at a moderate intensity showed a significant decrease in the POAG group. An increase in OPP was seen in the moderate intensity treadmill test exercise.
