    Abstract
    Background: Endometriotic cyst is a chronic gynecological disease often associated with chronic pelvic pain and infertility. Inflammatory markers such as Neutrophil-to-Lymphocyte Ratio (NLR) and Platelet-to-Lymphocyte Ratio (PLR) have potential utility in assessing inflammatory activity in endometriosis. This study aimed to compare NLR and PLR levels before and after surgery in patients with endometriotic cysts. Methods: This descriptive observational study with a pre-test post-test group design included 50 patients with endometriotic cysts undergoing surgery at Prof. Dr. Chairuddin P. Lubis Hospital, University of Sumatera Utara. Hematological parameters including neutrophils, lymphocytes, and platelets were collected pre- and post-surgery, followed by calculation of NLR and PLR. Data were analyzed using paired T-test with p<0.05 considered statistically significant. Results: The mean NLR significantly increased from 3.36 ± 3.16 pre-surgery to 11.60 ± 12.73 post-surgery (p=0.0001). Similarly, PLR levels significantly increased from 16,267.62 ± 14,978.09 to 39,461.75 ± 44,925.94 after surgery (p=0.001). These findings indicate a systemic inflammatory response following surgery in endometriotic cyst patients. Conclusion: There was a significant increase in NLR and PLR after surgery in patients with endometriotic cysts. NLR and PLR may serve as simple, accessible, and cost-effective inflammatory biomarkers in endometriosis management. Keyword : endometriosis cyst, NLR, PLR
