Non-Communicable Diseases (NCDs) are one of the major public health problems in Indonesia, including Medan City, contributing to high morbidity and mortality among the productive-age population. The Government, through the Ministry of Health, has implemented the Primary Health Care Integration (ILP) policy as an effort to strengthen promotive and preventive services, one of which is the implementation of NCD screening at community health centers (puskesmas). This study aims to analyze the implementation of the NCD screening program within the ILP framework at the Dentosa Baru Community Helath Center In Medan City, using an input,process, and output system approach. This research utilizes a qualitative method with a descriptive design. The informants consisted of the head of the community health center, the person in charge of the NCD program,posyandu (community health post) cadres, and productive age group community members. The findings show that in the input aspect, the NCD screening is supported by the availability of human resources, facilities, and budget, but there are still limitations in the number of staff, high workload, and dependency on the health center for providing screening tools. In the process aspect, the NCD screening is carried out through ILP posyandu activities for the productive age group, following a five-step service mechanism, which includes risk factor interviews, basic health examinations, counseling, and referrals. However, the screening implementation has not been comprehensive due to limited time and the simultaneous conduct of other health services. In the output aspect, the NCD screening program has raised community awareness of early detection of NCDs,but the coverage of screening for the productive age group has not met the established target. The conclusion of this study indicates that the implementation of the NCD screening program within the ILP framework at the Sentosa Baru Community Health Center in Medan has been carried out, but there is a need to strengthen resources, optimize comprehensive screening implementation, and improve cross-sector coordination to enhance the coverage and quality of services.
