Background : Death Mother caused by factors direct and indirect directly the pregnancy process, when give birth to or in 42 days after give birth to still become problem health significant in Indonesia, including in North Sumatra. This study analyzes Proportional as well as factor associated risks with death mother at Adam Malik Hospital Medan for the period 2022–2024. Data obtained from World Health The World Health Organization (WHO) in 2013 showed that the maternal mortality rate (MMR) was 210 per 100,000 live births, the MMR in developing countries was 230 per 100,000 live births and the MMR in developed countries was 16 per 100,000 live births. The main risk factors for pregnancy complications are preeclampsia / eclampsia , which contributes to approximately 24% of maternal deaths in Indonesia and is the second leading cause of maternal death in Indonesia; and heart disease in pregnancy, which causes maternal mortality in 10-15% of cases. 9 The main risk factor for childbirth complications is embolism, where if left untreated, DVT progresses to PE in 15-24% of patients and is potentially fatal in 15-30% of patients. 11 Postpartum / postpartum hemorrhage hemorrhage (PPH) is an emergency obstetric complications and causes approximately 35% of maternal deaths worldwide . Complications Postpartum increases the risk of maternal death. Complications during the postpartum period, particularly postpartum hemorrhage, can lead to maternal death if left untreated. Furthermore, 3% of maternal deaths are caused by postpartum peritonitis or post-abortion complications. Research Method : Research Analytic group retrospective done using recorded data medical all of it case death Mother 2022–2024 . A total of 64 samples analyzed , consisting of of 24 mothers who died and 40 mothers who survived . Analysis multivariate with regression logistics For determine factor risk significant (p < 0.05). Research Results : Of 1,286 deliveries , 24 deaths were found . mother ( proportion 1.33–2.25%). Dominant factor in the group die includes Hb <11 g/dL (87.5%), age pregnancy premature (76.2%), and parity at risk (47.8%). Causes direct call out is equal (68.8%) with accreta as the majority diagnosis . Analysis multivariate show that age pregnancy prematurity (OR: 8.634; 95% CI: 1.790 – 41.640), Hb < 11 gr/dl (OR: 5.413; 95% CI: 0.941 – 31.146) and parity risk of primipara and grandemultipara (OR: 3.421; 95% CI: 0.768 – 15.229). Increased ANC quality that focuses on detection early Pregnancy Keep going and pregnancy press , is required For lower maternal mortality .
