Background: The number of diseased vessels as well as the location and extension of coronary atherosclerotic lesions are predictors of cardiovascular events in patients with coronary artery disease (CAD). The SYNTAX II score, which integrates both anatomical and clinical variables, has been shown to provide better prognostic value. We report the predictive utility of the SYNTAX II score for major adverse cardiovascular events (MACE) within one year after hospitalization in patients with chronic coronary syndrome (CCS) and type 2 diabetes mellitus (DM-II) undergoing percutaneous coronary intervention (PCI). Objective: This study aimed to analyze the role of the SYNTAX II score as a predictor of MACE in CCS patients with DM-II undergoing PCI. Methods: This was an observational analytic study with a retrospective cohort design. Data were collected from June 2023 to August 2024 A total of 128 patients diagnosed with CCS and DM-II who underwent PCI were included. Bivariate and multivariate analyses, as well as survival probability using Kaplan-Meier curves, were performed to evaluate the relationship between the SYNTAX II score and MACE. Results: MACE occurred more frequently in the high SYNTAX II group (22 [34.4%]; P < 0.001). Both mortality and acute heart failure were significantly associated with the SYNTAX II score (P = 0.042 and P = 0.03, respectively). One-year MACE-free survival after PCI was significantly lower in CCS patients with DM-II and high SYNTAX II scores. Conclusion: The SYNTAX II score can be used as a predictive tool for MACE in CCS patients with T2DM undergoing PCI.en_US
