    Abstract
    The increasing elderly population in Indonesia is associated with a rising prevalence of periodontal disease. Saliva has great potential as a biomarker for detecting and monitoring various systemic health statuses, including the progression of periodontal disease. This study aimed to compare salivary alkaline phosphatase (ALP) levels and periodontal conditions between elderly males and females. The study was conducted using analytical observational method with a cross-sectional approach involving 88 elderly individuals (44 males and 44 females) living in Desa Suka Maju, Kabupaten Deli Serdang. The subjects periodontal condition was assessed using several parameters, including bleeding on probing (BoP), clinical attachment loss (CAL), pocket depth, and analysis of salivary alkaline phosphatase levels. The results showed that the mean salivary ALP level was 25.67±18.15 U/L for female subjects and 24.81±12.87 U/L for male subjects. Statistical analysis using the Mann-Whitney U test indicated no significant difference in salivary ALP levels (p=0.689), bleeding on probing (p=0.084), clinical attachment loss (p=0.311), and pocket depth (p=0.350) between elderly males and females. The conclusion of this study is that there is no significant difference in salivary alkaline phosphatase levels and periodontal conditions, specifically bleeding on probing, clinical attachment loss, and pocket depth, between elderly males and females
