    Abstract
    Background: Various complications can arise from uncontrolled diabetes mellitus (DM), one of which is diabetic nephropathy, which is the leading cause of kidney failure worldwide. Hyperglycemia activates various inflammatory pathways involving platelets. Albumin is also known to have anti-inflammatory effects. Therefore, the platelet-to-albumin ratio (PAR) can be used as an indicator of inflammation. Objective: To determine the value of PAR in predicting the occurrence of diabetic nephropathy in patients with type 2 diabetes mellitus. Methods: This study is a cross-sectional study involving type 2 DM patients at the Endocrinology outpatient clinic of Adam Malik Hospital. Patients who met the inclusion and exclusion criteria underwent complete blood tests, albumin, creatinine, eGFR, and albumin-creatinine ratio (ACR) tests. Results: There were significant differences in Hb, platelet, albumin, uric acid, creatinine, eGFR, ACR, and PAR parameters between the non-nephropathy and diabetic nephropathy groups. There was a significant correlation between PAR with ACR and albumin. PAR has an effect on the risk of diabetic nephropathy. PAR can predict the occurrence of diabetic nephropathy in type 2 DM patients with a cut-off of 7.05, with a sensitivity of 62.5% and specificity of 70.6%. Conclusion: PAR can predict the occurrence of diabetic nephropathy in type 2 DM patients.
