    Analysis of the Burden of Public Health Costs of PBPU JKN Participants Based on Household Expenditure in Southeast Aceh Regency

    Abstract
    Health is a basic human need that has a direct effect on the welfare and economic condition of households. Non-Wage Recipient Participants (PBPU) of the National Health Insurance are a group with a high level of vulnerability because they have to bear contributions independently, so the burden of health costs can affect the family's financial ability. This study aims to analyze the burden of health costs of PBPU-JKN participants based on household expenditure in Southeast Aceh Regency in 2025. The study used a cross sectional design with a descriptive approach on 35 respondents participating in PBPU-JKN who underwent treatment at H. Sahudin Kutacane Hospital. Primary data was collected through interviews using questionnaires, while secondary data was obtained from hospital medical records and BPJS Kesehatan documents. The variables analyzed included direct costs, indirect costs, household expenses, ability to pay (ATP), and the ratio of health burden to ATP. The analysis was carried out univariate to describe the distribution of each research variable. The results showed that the majority of respondents were of productive age (100%), high school education/equivalent (54,3%), and most of them worked in the informal sector such as farmers (31,4%) and traders (20,0%). The average direct costs borne by respondents ranged from IDR 524,657 to IDR 4,986,517, while indirect costs ranged from IDR 0-IDR 250,000. Total household expenses per month varied between IDR 390,000 – IDR 790,000. The average ATP of respondents ranged from IDR 20,000 to IDR 340,000, which indicates limited financial ability. The ratio of health burden to ATP for most respondents exceeded 40 percent, so it falls into the category of very heavy loads.
