Patient safety is an essential component in the quality of hospital services.
However, its implementation still faces challenges, especially in terms of
communication between medical personnel and incident recording systems. To
determine the effectiveness of the implementation of patient safety standards with a
focus on communication and incident recording in the inpatient room of
Bhayangkara Hospital Class II Medan. This study uses a descriptive qualitative
approach with in-depth interview techniques, non-participatory observation, and
documentation studies. There were 27 informants consisting of medical personnel,
patients, and hospital management. Communication between medical personnel
has not been fully effective due to the dominance of verbal communication and lack
of SBAR documentation. The incidentrecording system isstill manual and selective.
The culture of fear of reporting is also an obstacle in reporting incidents. The
implementation of patient safety standards is in the sufficient category, but still
requires improvement in aspects of training, digital reporting systems, and
organizational culture that supports openness and accountability.
