Background: Premature rupture of membranes (PROM) is the spontaneous rupture of the amniotic sac before the onset of labor. Premature rupture of membranes (PROM) is often caused by infection. Infection may lead to PROM through several mechanisms. Various inflammatory markers have been evaluated to detect infection associated with PROM. One of these markers is the neutrophil- to-lymphocyte ratio (NLR), which is derived from a differential white blood cell count. It is a simple and accessible laboratory parameter commonly used to reflect inflammation. Methods: This is an observational analytical study with a cross-sectional design to determine whether there is a relationship between the neutrophil-lymphocyte ratio and premature rupture of membranes at Adam Malik Hospital and Jejaring Hospital. This study was conducted from January 2025 after Ethical Clearance and research permit were issued until the sample size was met. The study sample was a portion of the population that met the inclusion and exclusion criteria. This study included 25 patients in the group with a gestational age of 34 to <37 weeks and 25 patients with normal term pregnancies. Data analysis used univariate analysis and the Independent T-test. Results: The results of this study found that the highest age group was 20-29 years old. The highest education level was high school. The highest occupation was housewife. The highest parity was among primigravidas. The Independent T- test statistical test yielded a p value <0.001. Conclusion: This study can be concluded that statistically there is a significant difference in NLR values with the incidence of premature rupture of membranes and normal term.
