Analisis Mitigasi Risiko Operasional Customer Identification Files (CIF) PT. Bank Sumut

Abstract

This thesis analyzes the mitigation of operational risk arising from errors in filling out the Customer Identification File (CIF) at PT. Bank SUMUT. This issue impacts the bank’s operations, cinpliance, finance, and reputation, with an average of 90.192 problematic CIFs and 23.505 duplicate CIFs per year, which has led to sanctions from the Otoritas Jasa Keuangan (OJK). The research aims to identify the causes of CIF errors originating from personnel, process, system, and external factors, and to formulate policy recommendations to minimize risk. The research methodology used is Failure Mode and Effect Analysis (FMEA) to proactively identify potential failures and Fault Tree Anlysis (FTA) to uncover their root causes. The FMEA results indicate that the cause of CIF errors is a complex interaction of system weakness, subomptimal processes, human resource limitations, and external pressure, with external and system and system factors contributing the greatest risk. One of the main faindings is the presence of external intervention that compromises the integrity of the officers. The conclusion of this research emphasizes the need for operational CIF risk mitigation through the implementation of strict reporting and monitoring systems, modernization of digital systems, updating of SOPs, and streengthening of human resource capacity to minimize detrimental impacts on the Bank
