    Pengaruh Buerger Allen Exercise Terhadap Sirkulasi Ektremitas Bawah pada Pasien Luka Kaki Diabetikdi Medan

    Abstract
    Luka kaki diabetes (LKD) merupakan salah satu komplikasi penyakit diabetes yang disebabkan oleh gangguan sirkulasi darah vena dan arteri. Buerger allen exercise adalah salah satu terapi modalitas dengan gerakan postural aktif untuk meningkatkan sirkulasi pembuluh darah vena dan arteri di ekstremitas bawah. Peningkatan sirkulasi kaki diukur menggunakan metode ankle brakial indeks (ABI). Tujuan penelitian ini untuk menilai pengaruh buerger allen exercise tehadap sirkulasi ektremitas bawah pasien LKD. Desain penelitian yang digunakan adalah quasi experiment dengan pendekatan pretest dan posttest without control. Jumlah sample penelitian ini sebanyak 43 responden, yang diambil dengan teknik consecutive sampling. Analisa data bivariat yang digunakan untuk mengetahui perbandingan nilai sebelum dan sesudah buerger allen exercise menggunakan uji wilcoxon. Hasil penelitian menunjukkan terdapat pengaruh yang signifikan antara nilai rata-rata ABI pada pasien LKD sebelum 0,84 dan sesudah 0,95 dengan (Pvalue=0.000). Intervensi buerger allen exercise dilakukan sebanyak 2 kali sehari, selama 3 minggu dengan perubahan posisi dan gaya gravitasi dapat membantu mengosongkan dan mengisi kolom darah secara bergantian pada pembuluh darah sedangkan kontraksi muskulus gastrocnemius dan otot plantar sebagai muscle pump mengaktivasi pembuluh melaju kejantung serta mencegah aliran balik. Latihan yang konsisten dapat membuka jalur sirkulasi collateral, sehingga meningkatkan distribusi nutrisi, suplai oksigen dalam darah menuju jaringan perifer. Berdasarakan hasil penelitian buerger allen exercise efektif meningkatkan sirkulasi sehingga kedepannya dapat direkomendasikan kepada perawat untuk mengajarkan kepada pasien yang mengalami gangguan sirkulasi ektremitas bawah pada pasien LKD.
     
    DFU (Diabetic Foot Ulcer) is one of the complications of diabetes caused by blood vein and artery circulation disorder of vena and artery. Buerger allen exercise is one of modality therapy with active postural movement which is aimed to increase blood vein and artery circulation in the lower extremity. The increase in foot circulation is measured by using Ankle Brochial Index (ABI). The objective of the research was to find out the influence of buerger allen exercise on the lower extremity circulation in DFU patients. The research used quasi experiment design with pretest-posttest without control approach. The samples were 43 respondents, taken by using consecutive sampling technique. The data were analyzed by using bivariate analysis in order to find out the mean difference between before and after buerger allen axercise, using Wilcoxon test. The result of the research showed that there was significant influence of the mean value of ABI in DFU patients before (0.84) and after (0.95) at p-value = 0.000). Buerger allen exercise intervention was done twice a day within 3 weeks with the change in position and gravitation power could help empty and fill up blood column in turn in blood vessel, while the contraction of musculus gastrocnemius and plantaris muscle as muscle pump activates vessel toward heart and prevents from reverse flow. Consistent exercise could open circulation collateral lane so that it increased nutrition distribution and supplied oxygen in blood to peripheral tissues. It was concluded that buerger allen exercise could effectively increase circulation so that it was recommended that nurses teach DFU patients who underwent lower extremity disorder.
