Luaran Klinis Satu Tahun Pasien Neglected Clubfoot yang Menjalani Operasi Soft Tissue Release Menggunakan Sistem Pirani Score

Abstract

Pendahuluan Neglected clubfoot adalah kelainan kongenital penyebab utama disabilitas lokomotor. Delapan puluh persen anak penderita clubfoot lahir di negara berkembang dan kebanyakan dari mereka tidak mendapatkan penanganan kesehatan yang baik. Salah satu tatalaksana yang dapat dilakukan pada pasien dengan neglected clubfoot adalah soft tissue release. Terdapat beberapa system penilaian derajat deformitas clubfoot diantaranya Pirani scoring system. Penelitian ini bertujuan mengetahui luaran klinis satu tahun pasien neglected clubfoot yang telah menjalani prosedur soft tissue release menggunakan penilaian skor Pirani. Metode Ini adalah penelitian cross sectional terhadap 17 pasien neglected clubfoot yang telah menjalani operasi soft tissue release satu tahun yang lalu, dengan membandingkan nilai skor Pirani sebelum dan satu tahun setelah operasi soft tissue release di Rumah Sakit Harapan. Data penunjang lain seperti usia, alasan keterlambatan berobat, lama penggunaan prostesis dan komplikasi paska operasi. Hasil Pada penelitian ini didapatkan 10 pasien perempuan dan 7 pasien laki-laki dengan rerata usia 69,06 ± 49.15. Deformitas pada kaki kiri sebanyak 13 kaki (59.1%) dan kaki kanan sebanyak 9 kaki (40.9%). Rerata total skor Pirani sebelum operasi 5,43 ± 1,03 dan rerata skor Pirani satu tahun setelah operasi 0,30 ± 0,40. Hal ini menunjukkan perbedaan hasil yang signifikan total skor Pirani sebelum dan satu tahun setelah operasi dimana p<0,0001 (p<0,05). Kesimpulan Luaran klinis satu tahun pasien neglected clubfoot yang ditatalaksana dengan prosedur soft tissue release menunjukkan perbaikan deformitas yang bermakna sesuai dengan sistem penilaian skoring Pirani.

Background Neglected clubfoot is the most common congenital problem leading to locomotor disability. Eighty percent of children born with clubfoot deformity are born in developing world and majority of them do not have access to appropriate medical care. One of the treatment for neglected clubfoot is soft tissue release. The severity of clubfoot may be assessed with a variety of methods where one of them is Pirani scoring system. This study aimed to evaluate the clinical outcome of neglected clubfoot patiens one year after the soft tissue release using Pirani scoring system. Methods This is cross sectional study of 17 patients who had been underwent soft tissue release a year ago. We compare Pirani score before soft tissue release and one year after soft tissue release in Harapan Hospital. The other datas that will be submitted are age, the reason of delayed treatment, term of prosthesis, and complication after surgery. Result The average age was 69,06 ± 49.15 months old. Deformity in left foot was 13 feet (59.1%) and right foot was 9 feet (40.9%) over 10 female and 7 male patients. The average total Pirani Score before soft tissue release is 5,43 ± 1,03 and the average total Pirani score one year after soft tissue release is 0,30 ± 0,40. This result shows significant improvement was obtained within total Pirani score before soft tissue release and one year after soft tissue release where p<0,0001 (p<0,05). Conclusion The one year clinical outcome of neglected clubfoot patient who underwent soft tissue release shows significant improvement using Pirani scoring system.
