Perbandingan Indeks Algofunctional Lequesne Terhadap Indeks WOMAC pada Pasien Osteoarthritis Lutut di RSUP. Haji Adam Malik Medan

Abstract

Latar belakang: Osteoartritis (OA) merupakan penyakit sendi yang paling sering di dunia. Lebih dari 80% manusia dengan usia diatas 55 tahun menderita osteoartritis pada salah satu sendi. Sendi lutut merupakan yang sangat penting karena tingginya prevalensi dari nyeri dan disabilitas. Dalam beberapa dekade terakhir indeks Western Ontario and McMaster (WOMAC) dan Lequesne algofunctional merupakan dua indeks yang paling sering digunakan dalam menilai tingkat keparahan osteosartritis lutut. Metodologi:Penelitian ini merupakan penelitian pontong lintang untuk mengetahui perbandingan validitias dan reliabilitas indeks WOMAC dan Lequesne pada pasien osteoartritis lutut. Dua puluh dua pasien di poliklinik Orthopedi dan Traumatologi RSUP. Haji Adam Malik Medan diminta untuk mengisi kedua kuisioner tersebut untuk kemudian dianalisis. Visual Analog Scale (VAS) dan data demografi juga dicatat sebagai bahan evaluasi adanya suatu korelasi. Reliabilitas dan internal consistency diukur dengan Cronbach’s alpha dan intra-class correlation. Validitas diukur dengan uji Pearson’s correlation coefficients. Untuk uji hipostesis digunakan Student’s t-test dengan nilai p< 0.05 dianggap signifikan. Hasil: Nilai Cronbach’s alpha dari indeks WOMAC 0.71-0.96 dan Lequesne 0.57-0.83. Nilai dari Intra-class correlation 0.46-0.98 pada WOMAC dan 0.20-0.92 untuk Lequesne. Validitas yang positif hanya ditemukan pada subskala nyeri pada indeks Lequesne jika dibandingkan dengan VAS. Untuk student’s t-test pada semua subskala menunjukkan nilai p > 0.05. Kesimpulan: Tidak ada perbedaan yang signifikan antara kedua kuisoner tersebut sehingga keduanya dapat digunakan untuk menilai tingkat keparahan dari osteoartritis lutut. Reliabilitas dari indeks WOMAC lebih dapat diterima dibandingkan indeks Lequesne.

Introduction: Osteoarthritis (OA) is the most common of joint disease throughout the world. Some studies estimate that over 80% of people over 55 years of age have osteoarthritis of at least one joint. Knee OA is the most important because of the high prevalence of pain and disability that they cause in older adults. In recent decades, the Western Ontario and McMaster (WOMAC) index and Lequesne algofunctional index are two of the most widely used outcome measures. Material: The aim of this cross sectional study is to comparatively evaluate the reliability and validity of the WOMAC and Lequesne index in patients with knee osteoarthritis. Twenty-two outpatients clinic in Orthopaedic Adam Malik General Hospital Medan were asked to complete the questionnaires. Visual Analog Scale (VAS) and demographic data were also recorded to evaluate further associations. Reliability and internal consistency was assessed by Cronbach’s alpha and intra-class correlation. Validity was assessed by Pearson’s correlation coefficients. Student’t t-test use to test the hypothesized with p < 0.05 was considered as statistically significant. Results: Cronbach’s alpha for WOMAC and Lequesne ranged from 0.71-0.96 and 0.57-0.83, respectively. Intraclass correlation for WOMAC and Lequesne ranged from 0.46-0.98 and 0.20-0.92, respectively. Postitive validity only found in pain subscale of Lequesne if compared to VAS. In student’s t-test, p > 0.05 was shows in all subscales of both indexes. Conclusion: There is no statitistically significant correlation between both indexes, so both indexes are reliable for evaluating knee osteoarthritis severity in Medan. Reliability of WOMAC more acceptable compared with the Lequesne.
