    Pengaruh Pemberian Ekstrak Etanol Daun Puguntano (Curanga Fel-Terrae Merr.) Terhadap Perbaikan Transduksi Sinyal Resistensi Insulin Post Reseptor pada Tikus Galur Wistar Model Diabetes Melitus Tipe 2

    Abstract
    Karakteristik utama Diabetes Melitus Tipe 2 (DMT2) adalah insulin resisten dan telah diketahui bahwa defek pada transduksi sinyal post reseptor merupakan penyebab utama resistensi insulin pada jaringan target. Penelitian ini bertujuan untuk membuktikan efek ekstrak etanol daun puguntano (Curanga fel-terrae [Merr.]) terhadap perbaikan transduksi sinyal resistensi insulin post reseptor (p38 MAPK, IRS, PI3K, GLUT-4) dan HOMA-IR pada tikus Wistar DMT2. Penelitian ini merupakan studi klinik acak, 48 tikus Wistar jantan diinduksi menjadi DMT2 dengan diet tinggi lemak (HFD) dan Streptozotocin (STZ) dosis rendah multipel. Kemudian secara acak dibagi menjadi dua kelompok, yaitu kelompok kontrol (n=24) dan kelompok perlakuan (n=24) yang mendapat ekstrak daun puguntano 200 mg/kgBB/tikus/hari. Berat badan, kadar gula darah (KGD) puasa, p38 MAPK, IRS, PI3K, GLUT-4 dan HOMA-IR tikus DMT2 diperiksa pada kelompok kontrol dan kelompok perlakuan setelah 10 hari. Data yang didapatkan dianalisa dengan uji Wilcoxon dengan tingkat kebermaknaan p<0,05. Setelah diinduksi menjadi DMT2, pada kelompok kontrol dan kelompok perlakuan terdapat peningkatan berat badan dan KGD puasa (p<0,001). Setelah 10 hari perlakuan dengan ekstrak daun puguntano, pada kelompok perlakuan terdapat penurunan bermakna berat badan (p<0,001), KGD puasa (p<0,001), insulin puasa (p=0,0005), HOMA-IR (p<0,001) dan peningkatan bermakna kadar p38 MAPK (p=0,0025), IRS (p=<0,001), PI3K (p=0,0015) dan GLUT-4 (p=0,0025) dibanding kelompok kontrol. Penelitian ini memperlihatkan bahwa ekstrak daun puguntano dapat memperbaiki transduksi sinyal resistensi insulin post reseptor melalui peningkatan kadar p38 MAPK, IRS, PI3K, GLUT-4 dan nilai HOMA-IR pada tikus DMT2.
     
    main characteristic of Type 2 Diabetes Mellitus (T2DM) is insulin resistance and it is known that defects in post receptor signaling are the major cause of insulin resistance in target tissue. This study aims to prove the effects of ethanol extract of puguntano leaves (Curanga fel-terrae [Merr.]) on improvement of post receptor insulin signaling of insulin resistance (p38 MAPK, IRS, PI3K, GLUT-4) and HOMA-IR in T2DM Wistar rat. This is a randomized clinical study, 48 male Wistar rat was induced to be T2DM with high fat diet (HFD) and multiple low dose streptozotocin (STZ) and then randomly to be assigned into two groups, ie. control (n=24) and treatment (n=24) group received puguntano leaves extract 200 mg/kg.bw/day. Body weight, fasting plasma glucose (FPG), p38 MAPK, IRS, PI3K, GLUT-4 and HOMA-IR of the T2DM rat were assessed in control group after establish T2DM and treatment group after 10 days treatment period. The data obtained were analyzed by Wilcoxon test with the level of significant p<0.05. After induction of T2DM, both the control and puguntano-treated rat groups showed elevated body weight and FPG level (p<0.001). After the 10 days treatment with puguntano leaves extract, in puguntano-treated group, a significant decrease was noted in body weight (p<0.001), FPG (p<0.001), fasting insulin (p=0.0005), HOMA-IR (<0.001) and a significant increase in p38 MAPK (p=0.0025), IRS (p=<0.001), PI3K (p=0.0015) and GLUT-4 (p=0.0025) levels compared with control group. This study indicated that puguntano leaves extract could improve post receptor insulin signaling of insulin resistance via increasing of p38 MAPK, IRS, PI3K, GLUT-4 levels and HOMA-IR value in T2DM rat.
