Pengaruh Pengingat Minum Obat (PMO) terhadap Tingkat Kepatuhan Pasien Diabetes Melitus Tipe 2 di Rumah Sakit Universtas Sumatera Utara Kota Medan

Abstract

Diabetes melitus (DM) adalah gangguan metabolisme yang ditandai dengan hiperglikemia disebabkan karena kekurangan produksi, aksi insulin, atau keduanya. Pengobatan DM memerlukan pengetahuan untuk meningkatkan kepatuhan. Kepatuhan diperlukan untuk mengontrol kadar glukosa darah dan mengurangi risiko komplikasi, dibutuhkan intervensi untuk meningatkan kepatuhan salah satunya adalah alat pengingat minum obat (PMO). Tujuan penelitian ini untuk mengetahui pengaruh PMO terhadap tingkat kepatuhan pada pasien Diabetes Melitus Tipe 2 di Rumah Sakit Universitas Sumatera Utara Medan. Penelitian Cohort prospective design ini dilakukan pada bulan Agustus – November 2018 di Rumah Sakit Universitas Sumatera Utara Medan dengan jumlah subjek 30 responden. Subjek penelitian ini one group pretest-posttest diberikan intervensi berupa aplikasi digital PMO pada smartphone mereka. Alat yang digunakan adalah kuesioner Diabetes Knowledge Questionnaire-24 (DKQ-24) untuk mengukur pengetahuan dan Morisky Medication Adherence Scale-8 (MMAS-8) untuk mengukur kepatuhan. Data yang diperoleh dikumpulkan dan dilakukan analisis menggunakan Microsoft Excel, statistik uji Spearman. Hasil penelitian ini menunjukkan tidak terdapat pengaruh tingkat pengetahuan dan kepatuhan terhadap jenis kelamin, usia, pendidikan, pekerjaan dan durasi DM. Berdasarkan hasil uji Spearman bahwa nilai Sig. (2-tailed) sebesar 0,301 (p≤0,05) diperoleh tingkat pengetahuan berkorelasi positif dengan tingkat kepatuhan meskipun dengan tingkat korelasi rendah sebesar 0,195. Berdasarkan hasil penelitian diperoleh kesimpulan bahwa Pengingat Minum Obat mempunyai pengaruh untuk meningkatkan pengetahuan dan kepatuhan pasien DM tipe 2.

Diabetes mellitus (DM) is a metabolism disorder characterized by hyperglycemia due to lack of production of insulin, insulin action, or in combination. DM requires knowledge to improve compliance. Compliance is needed to control blood glucose levels and reduce the risk of complications, intervention is needed to improve compliance, one of the interventions that can be used is a Medication Reminder Application (MRA). The purpose of this study was to determine the effect of MRA on the level of compliance in patients with Type 2 Diabetes Mellitus in the Universitas Sumatera Utara Hospital Medan. The Cohort prospective design study was conducted in August - November 2018 at the Universitas Sumatera Utara Hospital with a total of 30 respondents. The subject of this study was one group pretest-posttest was given intervention in the form of digital MRA on their smartphone. The tools used are a Diabetes Knowledge Questionnaire-24 (DKQ-24) to measure patient knowledge and Morisky Medication Adherence Scale-8 (MMAS-8) to measure patient’s compliance. The data obtained was collected and analyzed using Microsoft Excel, Spearman test statistics. The results of this study indicated that there is no impact on the level of knowledge and compliance to gender, age, education, occupation and duration of DM. Based on the Spearman Test the Sig (2-tailed) value was 0.301 (p≤0.05), the knowledge is positively correlated with the compliance rate of 0.195 which has a low correlate level. Based on the results, the conclusions from the Medication Reminder Application were obtained to increase the patient’s knowledge and compliance of type 2 DM patients.
