Pengaruh Drug Related Problems (DRPs) Terhadap Perubahan Kadar Gula Darah dalam Terapi Pasien Diabetes Melitus Tipe 2

Abstract

Diabetes melitus (DM) merupakan penyakit yang dapat menyebabkan terjadinya berbagai komplikasi sehingga mengharuskan pasien diabetes melitus tipe 2 (DMT2) mengkonsumsi obat selain obat antidiabetik dan berpotensi menyebabkan terjadinya drug related problems (DRPs) yang mempengaruhi outcome terapi termasuk perubahan kadar gula darah (KGD). Tujuan penelitian ini adalah untuk mengetahui pengaruh DRPs terhadap perubahan KGD pada pasien DMT2. Penelitian ini merupakan penelitian cross sectional prospektif yang dilakukan selama tiga bulan terhadap pasien rawat inap DMT2 dengan komplikasi di Rumah Sakit Umum Pusat (RSUP) Haji Adam Malik, Medan periode Juli-Oktober 2018. Identifikasi DRPs dengan menggunakan sistem klasifikasi DRPs Cipolle. Perubahan KGD dikategorikan menjadi hiperglikemia, hipoglikemia dan normoglikemia. Data yang diperoleh dianalisis secara statistik menggunakan Uji Chi-Square program SPSS versi 25. Hasil penelitian dari 81 pasien DMT2, 64,2% adalah laki-laki. Pasien paling banyak ditemui berusia 51-60 tahun yaitu sebesar 30,9%. Terapi yang paling banyak diberikan adalah kombinasi insulin kerja cepat dengan insulin kerja panjang (69,1%). Berdasarkan hasil penelitian ini, pasien dengan perubahan KGD hiperglikemia sebesar 27,2%; hipoglikemia sebesar 12,3% dan normoglikemia sebesar 60,5%. Tidak terdapat hubungan signifikan antara indikasi tanpa obat p = 0,064 dan obat tidak efektif p = 0,079 dengan perubahan KGD hiperglikemia dan terdapat hubungan signifikan antara dosis obat kurang p = 0,000 dengan perubahan KGD hiperglikemia. Terdapat hubungan signifikan antara interaksi obat dan reaksi obat merugikan p = 0,000 dengan perubahan KGD hipoglikemia. Berdasarkan hasil penelitian ini maka dapat disimpulkan bahwa terdapat pengaruh DRPs terhadap perubahan KGD dalam terapi pasien DMT2.

Diabetes mellitus (DM) can lead to the occurrence of macrovascular and microvascular complications.The incidence of complications requires that type 2 diabetes mellitus (T2DM) patients take drugs other than antidiabetic drugs which in turn can cause the occurrence of drug related problems (DRPs) that finally affect the blood glucose level (BGL). The purpose of this research was to investigate the effect of DRPs on BGL changes in the treatment of T2DM patients. This three-month prospective cross-sectional study was conducted to T2DM patients with complications hospitalized in Haji Adam Malik (HAM) Hospital, Medan, Indonesia period July to October 2018. DRPs were identified and classified using Cipolle DRP classification system. Blood glucose level changes were categorized as hyperglycemia, hypoglycemia and normoglycemia. The data obtained were analyzed by Chi-Square in SPSS program version 25 (p<0.05 implied that there was a significant relationship). This study involved 81 T2DM patients, 64.2% of the patients were male. Most (30.9%) of patients were at the age of 51-60 years. Combination of rapid and long acting insulins was the most frequently provided antidiabetic drugs (69.1%). The study proved that patients with hyperglycemia, 27.2%; hypoglycemia, 12.3%; normoglicemia, 60.5%. There was no significant relationship between hyperglycemia and indication without drug therapy (p = 0.064), ineffective provided drug (p = 0.079), and there was a significant relationship between irrational dose (p = 0.000) and hyperglycemia. There was a significant relationship between drug interaction (p = 0.000) and hypoglycemia. The present study concluded that DRPs caused BGL changes in the treatment of T2DM patients.
