Korelasi Kualitas Tidur dengan Kadar Interleukin-6 Serum pada Pasien Hemodialisis Regular

Abstract

Latar Belakang: Gangguan tidur merupakan hal yang umum dijumpai pada pasien dengan penyakit ginjal tahap akhir (PGTA).Pasien dialisis yang mengalami permasalahan tidur mencapai 50-80%. Kadar sitokin seperti interleukin-6 (IL-6) secara signifikan mempengaruhi tidur baik secara positif dan negatif (efek somnogenik) dan sering dijumpai meningkat pada pasien hemodialisis.Gangguan tidur dan inflamasi sangat umum dijumpai pada pasien hemodialisis. Kedua kondisi ini berhubungan dengan peningkatan mortalitas. Tujuan : Mengetahui hubungan antara kualitas tidur dengan kadar interleukin-6 serum pada pasien hemodialisis reguler Metode : Penelitian ini dilakukan secara observasional dengan metode potong lintang. Penelitian dimulai pada bulan Agustus – September 2015 pada pasien di RSUP. H. Adam Malik dan RS. Dr. Pirngadi Medan. Dari 54 pasien yang menjalani HD, 34 pasien memenuhi syarat dalam penelitiandilakukan anamnesis dan pemeriksaan laboratorium.Pasien diminta untuk mengisi kuesioner Pittsburh Sleep Quality Index (PSQI) yang berisi 19 pertanyaan. Hasil : Dari 34 pasien yang menjalani hemodialisis reguler didapatkan 100% mengalami kualitas tidur yang jelek dengan rata-rata skor PSQI adalah 10,88 ± 2,705 dan kadar rata-rata IL-6 adalah 8,497 ± 6,140. Hubungan (kolerasi) PSQI terhadap IL-6 diukur dengan uji Pearson dimana nilai korelasi (r) adalah 0,503. Diketahui nilai p-value = 0,002 < 0,05, maka korelasi antara PSQI dan IL-6 signifikan secara statistika. Kesimpulan : Terdapat korelasi positif dengan kekuatan kuat (r=0,503) antara tingkat skor PSQI terhadap kadar IL-6 pada pasien penyakit ginjal kronik dengan hemodialisis reguler.

Background : Sleep disturbance is common in patients with end-stage renal disease (ESRD). Dialysis patients who experience sleep problems reach 50-80%. Levels of interleukin-6 (IL-6) significantly affect sleep both positively and negatively (somnogenic effects) and are often seen elevated in hemodialysis patients. Sleep disturbance and inflammation are very common in hemodialysis patients. Both of these conditions are associated with increased mortality. Objective : To determine the association between sleep quality and interleukin-6 levels in patients with regular hemodialysis Methods : This research was conducted by observational with the method by cross section. The study began in August - September 2015 in patients at the RSUP. H. Adam Malik and RS. Dr. Pirngadi Medan. Of the 54 patients undergoing HD, 34 patients were eligible in the study for anamnesis and performed laboratory tests. Patients were asked to fill out the Pittsburh Sleep Quality Index questionnaire containing 19 questions. Results : Of the 34 patients who underwent regular hemodialysis, 100% experienced poor sleep quality with an average PSQI score of 10.88 ± 2.705 and the mean IL-6 level was 8.497 ± 6.140. PSQI correlation relationship to IL-6 was measured by Pearson test where the correlation value (r) was 0,503. Given p-value = 0.002 <0.05, the correlation between PSQI and IL-6 is statistically significant. Conclusion : There was a positive correlation with strong strength (r = 0,503) between PSQI score level on IL-6 levels in patients with chronic kidney disease with regular hemodialysis.
