Pemantauan Efektivitas Terapi Spironolakton dan Kombinasinya Dengan Furosemida Terhadap Penderita Sirosis Hati yang Disertai Asites di RSUP H. Adam Malik Medan

Abstract

Spironolactone and furosemide were diuretics used to overcome liver cirrhosis with ascites, but there were not case studies to distinguish the effectiveness between spironolactone and its combination with furosemide in cases of liver cirrhosis accompanied by ascites in RSUP. H Adam Malik Medan. This study aimed to determine the effectiveness of sprinolactone therapy and its combination with furosemide to liver cirrhosis patients accompanied ascites in RSUP H. Adam Malik Medan. This research was by Cross Sectional Prospect method. of 26 patients in RSUP H Adam Malik Medan, divided into 2 groups. The first group consisted by 9 patients between the ages of 39-65 years, were given spironolactone. the second group consisted by 17 patients aged 20-81 years were given a combination with furosemide. The sample data were tested with statistics using Mann Whitney U, SPSS Program version 22. Based on Mann Whitney U statistic test, the value of Asymp sig (2 tailed) 0.435 mEq for urine Na/K at time, and Asymp sig value (2 tailed) 0.628 mEq 24 hour urine sodium examination, based on the value it was concluded that there was not significant difference between sprinolactone with its combination with furosemide. Eventhough the significant value were 0.628 mEq and 0.435 mEq (p>0.05). This study showed that the combination of spironolactone with furosemide was better compared to sprinolactone. although there was not statistically significant difference if seen from the comparison of 24 hours of urine sodium value with a normal value of 78 mEq.

Spironolakton dan furosemida adalah diuretik yang digunakan untuk mengatasi penyakit sirosis hati yang disertai asites, namun belum ada studi kasus untuk membedakan keefektifan antara spironolakton dan kombinasinya dengan furosemida pada kasus sirosis hati yang disertai asites di RSUP H. Adam Malik Medan. Penelitian ini bertujuan untuk mengetahui efektivitas terapi sprinolakton dan kombinasinya dengan furosemida terhadap pasien sirosis hati yang disertai asites di RSUP H. Adam Malik Medan. Penelitian ini dilakukan dengan metode Cross Sectional Prospektif. Terhadap 26 pasien di RSUP H Adam Malik Medan, dibagi menjadi 2 kelompok. Kelompok pertama berjumlah 9 pasien, berumur antara 39-65 tahun, diberi spironolakton. Kelompok kedua 17 pasien, berumur antara 20-81 tahun diberi spironolakton kombinasi dengan furosemida. Data sampel diuji dengan statistik menggunakan Mann Whitney U, Program SPSS versi 22. Berdasarkan hasil uji statistik Mann Whitney U diperoleh nilai Asymp sig (2 tailed) 0,435 mEq untuk pemeriksaan Na/K urin sewaktu, dan nilai Asymp sig(2 tailed) 0,628 mEq untuk pemeriksaan natrium urin 24 jam. Berdasarkan nilai tersebut disimpulkan bahwa tidak ada perbedaan yang signifikan antara sprinolakton dan kombinasinya dengan furosemida. Hasil pada nilai Asymp sig (2 tailed) lebih besar dari 0,05 yaitu 0,628 mEq dan 0,435 mEq. Penelitian ini menunjukkan bahwa kombinasi spironolakton dengan furosemida lebih baik dibandingkan sprinolakton tunggal.Walaupun secara statistik tidak ada perbedaan yang signifikan Jika mengacu pada perbandingan nilai natrium urin 24 jam dengan nilai normal 78 mEq.
