    Hubungan Inklinasi dengan Ketebalan Tulang Alveolar Gigi Insisivus Sentralis Maksila di Klinik Ortodonti RSGMP FKG USU

    Abstract
    Background: In an orthodontic treatment, the labio-lingual movement of the anterior teeth is required to improve the sagittal relationship of the maxillary and mandibular arch in order to achieve a more harmonious profile. However, excessive force to the teeth movement could lead to iatrogenic effect, therefore an evaluation of tooth supporting tissue is essential prior to treatment. The purpose of this research is to understand the relationship between inclination and the thickness of the alveolar bone of the maxillary central incisors. Materials and methods: The total sample in this research is 135 lateral cephalograms, divided into three groups : normal inclination, proclination and retroclination respectively. The labio-lingual inclination of the central incisors is determined based on the angle of central maxillary incisor towards Sella-Nasion ( SN Plane). The labial and lingual thickness of the alveolar bone is determined by a line drawn perpendicular to the axis of the tooth, measured in linear on each 1/10 level along the root of teeth, from cement-enamel junction until the root of apical the central maxillary incisor. Kruska-Wallis test is used to determine the relationship between the inclination and the alveolar bone thickness of central maxillary incisors . Results: There is a significant difference between the thickness of the labial and lingual alveolar bone with the inclination. p < 0,005. There is a significant relationship between the inclination of the alveolar bone thickness with labial and lingual root apical teeth on central maxillary incisivus. Conclusion: The retroclined central maxillary teeth tend to have thinner labial alveolar bone support especially in root area and must be taken into consideration. The prevalence of fenestration in retroclined is higher than normal group. Whereas there was no fenestration in proclined group.
     
    Latar Belakang: Dalam perawatan ortodonti pergerakan labio-lingual pada gigi anterior diperlukan untuk memperbaiki hubungan sagital lengkung maksila dan mandibula sehingga tercapai profil pasien yang harmonis. Namun, pergerakan gigi yang berlebihan dapat menyebabkan gejala iatrogenik. Dengan demikian, perlu dilakukan evaluasi terhadap jaringan pendukung gigi sebelum perawatan. Tujuan penelitian: adalah untuk mengetahui hubungan inklinasi dengan ketebalan tulang alveolar pada gigi insisivus sentralis maksila. Bahan dan Cara: Sampel penelitian adalah 135 foto sefalometri lateral, dibagi atas tiga kelompok yaitu inklinasi normal, proklinasi dan retroklinasi. Inklinasi gigi insisivus sentralis maksila dalam arah labio lingual dihitung berdasarkan sudut insisivus sentralis makssila terhadap Sella-Nasion (SN). Ketebalan tulang alveolar labial dan lingual ditentukan dengan menarik garis tegak lurus terhadap sumbu gigi yang diukur secara linier setiap 1/10 level sepanjang akar dari cemento enamel junction sampai apikal akar gigi insisivus sentralis maksila. Selanjutnya dilakukan uji Kruskal Wallis untuk mengetahui hubungan inklinasi dengan ketebalan tulang alveolar gigi insisivus sentralis maksila.. Hasil: Terdapat perbedaan ketebalan tulang alveolar labial dan lingual yang signifikan pada inklinasi yang berbeda (p<0.005). Ada hubungan yang signifikan antara inklinasi dengan ketebalan tulang alveolar labial dan lingual pada apikal akar gigi insisivus sentralis maksila. Kesimpulan: Gigi insisivus sentralis maksila yang retroklinasi cenderung memiliki dukungan tulang alveolar labial yang tipis pada apikal akar labial sehingga harus dikelola secara hati-hati. Fenestrasi lebih sering terjdi pada kelompok retroklinasi dari pada kelompok normal. Pada kelompok proklinasi tidak dijumpai adanya fenestrasi.
