    Peran Polimorfisme Gen Reseptor Vitamin D FokI rs2228570, ApaI rs7975232, TaqI rs731236 dan Antibodi IgM Anti-PGL-1 Serta Faktor-Faktor Risiko untuk Mengalami Kusta pada Narakontak Serumah

    Abstract
    Background: Leprosy is a chronic infectious disease with dermatologic and neurologic manifestations, which is caused by Mycobaterium leprae and can be transmitted through inhalation, direct skin contact, ingestion, feto-maternal and traumatic inoculation. Some studies have reported that vitamin D and its receptor gene polymorphisms (VDR polimorphisms) play a significant role in the risk of leprosy. Other risk factors such as age, gender, body mass index, family relation, number of family members, duration of contact and house’s ventilations, are known to affect the risk of this disease. Aim: To evaluate the role of FokI rs2228570, ApaI rs7975232, TaqI rs731236 vitamin D receptor gene polymorphisms, anti-PGL-1 IgM antibody and risk factors of leprosy in household contacts. Subject and Methods: This is an analytical study with case control design that involved 52 leprosy patients with 95 household contacts and controls, respectively, from all referral center in North Sumatera, Indonesia. History taking, physical and blood examinations, and house survey were done towards all participants. Evaluation of anti- PGL-1 IgM antibody titer was done using ELISA. PCR-RFLP was used to determine FokI rs2228570, ApaI rs7975232, TaqI rs731236 vitamin D receptor gene polymorphisms. Bivariate statistical analysis using Chi-square test was done to determine the relation between FokI rs2228570, ApaI rs7975232, TaqI rs731236 VDR gene polymorphisms and all risk factors with anti-PGL-1 IgM antibody titer. Multivariate analysis using double logistic regression was done to determine the most affecting risk factor in leprosy among household contacts, with p<0,05 indicating significant result. Results: There were not significant relation between FokI rs2228570 (p=0.342), ApaI rs7975232 (p=0.255), TaqI rs731236 (p=0.895) VDR gene polymorphisms with anti- PGL-1 IgM antibody titer among household contacts of leprosy patients. Among 7 risk factors, only the number of family members had significant effect with anti-PGL-1 IgM antibody titer in household contacts (p=0.045). Multivariate analysis revealed no significant effect between number of family members and ventilation of the house. Therefore, the most affecting risk factor in leprosy among household contacts were not able to be determined. Conclusion: There were no role of FokI rs2228570, ApaI rs7975232, TaqI rs731236 vitamin D receptor gene polymorphisms, anti-PGL-1 IgM antibody and risk factors that can be found affecting leprosy incidence among household contacts.
     
    Latar belakang: Kusta merupakan penyakit dermatologik dan neurologik yang kronik, disebabkan infeksi patogen Mycobaterium leprae (M.leprae) yang ditularkan melalui inhalasi, kontak langsung kulit, transmisi in uteri, melalui saluran cerna, dan inokulasi pascatrauma. Pada beberapa penelitian sebelumnya diketahui bahwa vitamin D dan polimorfisme gen reseptor (polimorfisme VDR) berperan dalam risiko untuk terinfeksi kusta. Selain itu, terdapat faktor risiko lain yang ikut berperan seperti usia, jenis kelamin, indeks massa tubuh, hubungan keluarga, jumlah anggota keluarga, lama kontak dan ventilasi. Tujuan: Mengetahui peran polimorfisme gen reseptor vitamin D FokI rs2228570, ApaI rs7975232, TaqI rs731236, antibodi IgM anti-PGL-1 dan faktor-faktor risiko untuk mengalami kusta pada narakontak serumah. Subjek dan metode: Penelitian ini merupakan suatu studi analitik dengan rancangan penelitian kasus kontrol yang melibatkan 52 orang pasien kusta dan masing-masing 95 orang narakontak serumah pasien kusta dan kontrol yang didapatkan dari berbagai pusat pelayanan kesehatan di provinsi Sumatera Utara. Kemudian dilakukan wawancara, pemeriksaan fisik, pemeriksaan darah dan kunjungan rumah dan dilanjutkan pemeriksaan titer antibodi IgM anti-PGL-1 dengan ELISA dan pemeriksaan polimorfisme gen reseptor vitamin D FokI rs2228570, ApaI rs7975232 dan TaqI rs731236 menggunakan metode PCR-RFLP. Analisis statistik bivariat menggunakan uji Chi-square dilakukan untuk melihat hubungan antar variabel polimorfisme gen reseptor vitamin D FokI rs2228570, ApaI rs7975232, TaqI rs731236 dan semua faktor risiko yang diteliti dengan titer antibodi IgM anti-PGL-1. Kemudian, analisis multivariat regresi logistik ganda dilakukan untuk menentukan faktor risiko kusta yang paling berpengaruh pada narakontak serumah. Nilai p<0,05 dianggap signifikan secara statistik. Hasil: Tidak terdapat hubungan antara polimorfisme gen reseptor vitamin D FokI rs2228570 (p=0,342), ApaI rs7975232 (p=0,255), TaqI rs731236 (p=0,895) dengan titer antibodi IgM anti-PGL-1 pada narakontak serumah. Di antara 7 faktor risiko, hanya jumlah anggota keluarga dengan titer antibodi IgM anti-PGL-1 pada narakontak serumah yang memiliki pengaruh yang signifikan (p=0,045). Analisis multivariat menggunakan regresi logistik ganda tidak menunjukkan adanya pengaruh bermakna antara jumlah anggota keluarga dengan syarat ventilasi (p>0,05), sehingga tidak dapat ditentukan faktor risiko yang paling berpengaruh untuk mengalami kusta pada narakontak serumah. Kesimpulan: Tidak ditemukan peranan polimorfisme gen reseptor vitamin D FokI rs2228570, ApaI rs7975232, TaqI rs731236, antibodi IgM anti-PGL-1 dan faktor-faktor risiko untuk mengalami kusta pada narakontak serumah.
