    Pengaruh Sodium Hipoklorit, Edta dan Larutan Kitosan Molekul Tinggi Terhadap Tingkat Denaturasi dan Erosi Kolagen Dentin Saluran Akar (Penelitian In Vitro)

    Abstract
    Root canal treatment is carried out to maintain the function of the teeth that are currently or have suffered a pulp injury or inflammation. The focus of root canal treatment is directed at the stages of cleaning and shaping through the root canal cleaning procedure using a chemomechanical approach, which is a combination of instrumentation with metal-based tools and irrigation with chemical solutions, and root canal filling procedures. Various methods are carried out to increase the work effect of the irrigation solution including sonic activation. Side effects caused by a combination of irrigation chemical solutions encourage many researchers to look for alternative materials that are safer, more biocompatible and more profitable. Chitosan is the most promising ingredient. Objective: The purpose of this study is to compare the differences in the use of sodium hypochlorite irrigation solutions, EDTA and chitosan as irrigation solutions that can protect dentin collagen from denaturation and erosion. Methods: Thirty-two single-rooted premolar teeth were divided into eight groups, each containing four samples. Samples were treated with root canal treatment with a combination of sodium hypochlorite solution, EDTA and high molecular chitosan with conventional irrigation techniques and sonic activation techniques. Calcium degradation was measured by EDX machine, the level of denaturation and erosion was measured by histopathology and FTIR. Results: The highest calcium degradation in the group with a combination of sodium hypochlorite, EDTA and high molecular chitosan activation methods. The lowest denaturation and erosion of collagen is found in samples with a combination of sodium hypochlorite, EDTA and high molecular chitosan activation methods. Conclusion: High molecular chitosan 0.2% can protect dentin collagen from denaturation and erosion of root canal irrigation solution.
     
    Perawatan saluran akar dilakukan untuk mempertahankan fungsi gigi geligi yang sedang ataupun pernah mengalami cedera pulpa ataupun terinflamasi. Fokus perawatan saluran akar diarahkan pada tahapan pembersihan dan pembentukan (cleaning and shaping) melalui prosedur pembersihan saluran akar menggunakan pendekatan kemomekanis, yakni kombinasi instrumentasi dengan alat berbahan metal dan irigasi dengan larutan kimia, dan prosedur pengisian saluran akar. Berbagai metode yang dilakukan untuk meningkatkan efek kerja dari larutan irigasi diantaranya adalah dengan aktivasi sonik. Efek samping yang disebabkan oleh kombinasi larutan kimia irigasi mendorong banyak peneliti untuk mencari bahan alternatif yang lebih aman, lebih biokompatibel dan lebih menguntungkan. Kitosan merupakan bahan yang paling menjanjikan. Tujuan: Tujuan dari penelitian ini adalah untuk membandingkan perbedaan penggunaan larutan irigasi sodium hipoklorit, EDTA dan kitosan sebagai larutan irigasi yang dapat menjaga kolagen dentin dari denaturasi dan erosi. Metode: Tiga puluh dua gigi premolar berakar tunggal dibagi menjadi delapan kelompok, masing-masing berisi empat sampel. Sampel diberi perlakuan perawatan saluran akar dengan kombinasi larutan sodium hipoklorit, EDTA dan kitosan molekul tinggi dengan teknik irigasi konvensional dan teknik aktivasi sonik. Degradasi kalsium diukur dengan mesin EDX, tingkat denaturasi dan erosi diukur dengan histopatologi dan FTIR. Hasil: Degradasi kalsium tertinggi di kelompok dengan kombinasi sodium hipoklorit, EDTA dan kitosan molekul tinggi metode aktivasi. Denaturasi dan erosi kolagen paling rendah dijumpai di sampel dengan kombinasi sodium hipoklorit, EDTA dan kitosan molekul tinggi metode aktivasi. Kesimpulan: Kitosan molekul tinggi 0,2% mampu menjaga kolagen dentin dari denaturasi dan erosi larutan irigasi saluran akar.
