Pengaruh Pemberian Ekstrak Ethanol Daun Puguntano (Curanga fel-terrae Merr.) Terhadap Viabilitas Sel, Ekspresi Tumor Necrosis Factor-α dan Interleukin-1α pada Biakan Matriks Kolesteatoma Penderita Otitis Media Supuratif Kronis Tipe Atikoantral

Abstract

Otitis Media Supuratif Kronis kronis dengan atau tanpa kolesteatoma adalah masalah utama yang sudah bersifat global dan itu menjadi beban terutama di negara-negara berkembang. Penelitian-penelitian menemukan bahwa ekstrak etanol daun Puguntano yang diperoleh dengan metode perkolasi dan sokletasi memiliki kandungan fitokemikal yang memberikan efek anti-inflamasi. Tujuan dari penelitian ini adalah untuk menyelidiki efek dari ekstrak etanol dan Puguntano pada viabilitas keratinosit kolesteatoma, ekspresi TNF- α dan IL-1α . Studi ini merupakan penelitian eksperimental in vitro yang mencakup 8 kelompok kultur keratinosit kolesteatoma yang terdiri atas satu kelompok yang tidak diberi ekstrak etanol daun puguntano, tiga kelompok kontrol positif diberi konsentrasi deksametason yang berbeda, dan empat kelompok yang diberi konsentrasi berbeda dari ekstrak etanol daun Puguntano. Dari hasil penelitian didapatkan viabilitas pada kelompok kontrol menunjukkan rerata tertinggi jauh berada di atas kelompok yanng diberikan deksametason dan puguntano dengan rerata 45,6. Dengan menggunakan uji post hoc tampak bahwa seluruh kelompok deksametason dan puguntano berbeda signifikan dengan kelompok kontrol berdasarkan viabilitas keratinosit kelesteatoma (p<0,001). Rerata TNF α tertinggi terdapat pada kelompok kontrol dengan rerata 93,94 pg/ml. Pada kelompok yang diintervensi dengan pemberian deksametason rerata TNF α berkisar antara 80,87 pg/ml pada deksametason dosis 100 sampai 84,22 pg/ml pada deksametason dosis 1. Pada kelompok perlakuan dengan pemberian puguntano memiliki nilai TNF α terendah dengan rerata 78,25 pg/ml pada kelompok P4 dan P8. Rerata Interleukin 1 tertinggi terdapat pada kelompok kontrol dengan rerata 177,36. Pada kelompok yang diintervensi dengan pemberian deksametason rerata Interleukin 1 berkisar antara 19,03 pada deksametason dosis 100 sampai 90,13 pada deksametason dosis 1. Pada kelompok perlakuan dengan pemberian puguntano memiliki nilai interleukin 1 dengan rerata terendah pada kelompok P8 dengan rerata interleukin 1 sebesar 6,95. Kesimpulan Penelitian ini menunjukkan bahwa ekstrak daun Puguntano memiliki efek yang sama dengan deksametason dalam hal menekan viabilitas sel dan juga menurunkan ekspresi TNF- α dan IL-1α. Dengan demikian, ekstrak etanol daun Puguntano dapat digunakan sebagai alternatif untuk tatalaksana OMSK tipe Atikoantral.

Chronic suppurative otitis media with or without cholesteatoma is a major global problem and it is a burden especially in developing countries. Studies have found that the ethanol extract of Puguntano leaves obtained by the percolation and soccletation methods has phytochemical content which provides anti-inflammatory effects. The aim of this study was to investigate the effects of ethanol and Puguntano extracts on the viability of cholesteatoma keratinocytes, TNF-α expression and IL-1α. This study was an in vitro experimental study that included 8 groups of cholesteatoma keratinocyte cultures consisting of one group without puguntano leaf ethanol extract, three positive control groups were given different dexamethasone concentrations, and four groups were given different concentrations of ethanol extract of Puguntano leaf. Then from the results obtained viability in the control group showed the highest mean was far above the group given dexamethasone and puguntano with a mean of 45.6. By using the post hoc test it appears that the entire dexamethasone and puguntano groups differed significantly from the control group based on the viability of keratinocyte kelesteatoma (p <0.001). A statistically significant reduction in procalcitonone levels (p <0.001) and a number of bacterial colonies (p <0.001) was found in four groups. The highest Α TNF was found in the control group with a mean of 93.94 pg / ml. In the group intervened by administration of dexamethasone, the average Α TNF ranged from 80.87 pg / ml at dexamethasone at a dose of 100 to 84.22 pg / ml in dexamethasone dose 1. In the treatment group with puguntano administration, the lowest Α TNF value was 78.25 pg / ml in groups P4 and P8. The highest interleukin 1 was found in the control group with a mean of 177.36. In the group intervened by the administration of dexamethasone average Interleukin 1 ranged from 19.03 in dexamethasone doses of 100 to 90.13 in dexamethasone dose 1. In the treatment group with puguntano administration, interleukin 1 had the lowest value in group P8 with average interleukin 1 of 6 , 95. Conclusion This study shows that Puguntano leaf extract has the same effect as dexamethasone in terms of suppressing cell viability and also decreasing TNF-α and IL-1α expression. Thus, ethanol extract of Puguntano leaves can be used as an alternative for the management of Atikoantral type CSOM.
