    Simulasi Ketahanan Fraktur dan Distribusi Tegangan pada Gigi Pasca Perawatan Endodonti yang Direstorasi dengan Pasak Prefabricated dan Mahkota Penuh dengan Desain Ferrule yang Berbeda: Finite Element Analysis (FEA)

    Abstract
    Restoration of teeth after endodontic treatment often presents challenges because post-treatment endodontic teeth have the potential to fracture. Fibre post and core restorations have been used as endodontic treatment restorations because they can distribute occlusal pressure to prevent tooth fractures. The mechanical effect of the ferrule on the cervical gingival area is approach to strengthen teeth after post restoration. Ferrule is claimed to prevent the destruction of the supporting tooth roots and helps in providing resistance to the release force and prevents fractures. A minimum ferrule height of 1-2 mm is required to achieve a protective effect. Destructive mechanical tests are important for biomechanical analysis of teeth and dental restorative materials, however, these tests have a limited capacity to explain the stress and strain relationship Finite element analysis (FEA) has been developed to obtain maximum results for biomechanical analysis. This simulation was carried out to compare the effect of the design of the height of the ferrule on fracture resistance and stress distribution. The first five maxillary 3D incisors were constructed on a computer (AutoCAD 2016 software) with ferrule heights: 0mm ferrule, 0.5mm, 1mm, 1.5mm, and 2mm. Then meshing and input mechanical properties were carried out followed by given a load force of 25 N and 170 N at an angle of 135º in the palatal area. The results of this simulation showed that increasing the height of the ferrule can increased the fracture resistance of the teeth after endodontic treatment by reducing the deformation of the model. Ferrule had also been shown to reduce stress concentration values in the labial cervical region. It is also had seen that the ferrule can enlarge the strain in the cervical palatal area but the palatal ferrule wall can withstand non-axial loads from the palatal side.
     
    Restorasi gigi pasca perawatan endodonti sering menimbulkan tantangan karena gigi pasca perawatan endodonti berpotensi fraktur. Restorasi pasak fiber dan inti sudah digunakan sebagai restorasi perawatan endodonti karena mampu mendistribusikan tekanan oklusal sehingga dapat mencegah terjadinya fraktur gigi. Efek mekanik ferrule pada daerah servikal gingiva adalah salah satu pendekatan untuk memperkuat gigi setelah restorasi pasak. Ferrule diklaim untuk mencegah hancurnya akar gigi penyangga dan membantu dalam memberikan perlawanan terhadap gaya pelepasan dan mencegah fraktur. Tinggi ferrule minimal 1-2 mm yang diperlukan untuk mencapai efek perlindungan. Uji mekanis dekstrutif penting untuk analisis biomekanik gigi dan bahan restorasi gigi, namun, uji ini memiliki kapasitas terbatas untuk menjelaskan hubungan tegangan dan regangan Finite element analysis (FEA) telah dikembangkan untuk memperoleh hasil yang lebih maksimal untuk analisis biomekanik. Simulasi ini dilakukan untuk membandingkan efek desain ketinggian ferrule terhadap ketahanan fraktur dan distribusi tegangan. lima model gigi insisivus pertama maksila 3D yang dikontruksi pada komputer (software AutoCAD 2016) dengan ketinggian ferrule: 0mm ferrule, 0,5mm, 1mm, 1,5mm dan 2mm. Kemudian dilakukan meshing dan input mechanical properties dilanjutkan pemberian beban 25 N dan 170 N pada sudut 1350 didaerah palatal. Hasil simulasi ini menunjukkan dengan bertambahnya tinggi ferrule dinyatakan dapat meningkatkan ketahanan fraktur gigi pasca perwatan endodonti dengan menurunkan deformasi dari model. Ferrule juga terbukti dapat menurunkan nilai konsentrasi tegangan pada daerah servikal labial. Selain itu juga terlihat bahwa ferrule dapat memperbesar regangan pada daerah servikal palatal dentin namun dinding palatal ferrule dapat menahan beban non-aksial dari sisi palatal.
