Hubungan Perubahan Rotasi Mandibula Terhadap Posisi dan Angulasi Kondilus pada Maloklusi Kelas II Skeletal Setelah Perawatan Ortodonti

Abstract

Background: Treatment of skeletal Class II malocclusions may cause changes in the occlusal plane, position of the mandible and it’s rotation which in turn will affects the facial vertical dimension and profile. Changes in the mandible rotation which often happens during the treatment of skeletal class II malocclusions affects the position and the angulation of the mandible condyle. However the changes in the mandible’s rotation during orthodontic treatment is still controversial. Purpose: To determine the relationship between changes in mandible’s rotation and the position and angulation of the condyle in skeletal class II patients post orthodontic treatment. Method: This study’s design was cross sectional using samples from lateral cephalographic radiographs of 30 patients pre and post orthodontic treatment with or without extraction. Changes in mandible’s rotation was measured using MP SN angle while the displacement was measured using the distance of Co-Tc, Co-TVertical and the condyle’s angle was assessed with Co-T-TVertical. Result: There was no significant changes in the mandible’s rotation pre and post treatment with or without extraction (p>0.05). There was a significant change in the mandible’s angulation pre and post treatment with extraction (p<0.05). There was a strong correlation between changes in mandible’s rotation and the condyle’s position (Co-TVertical) in patients without extraction (r>0.5). There was no significant correlation between mandible’s rotation and the position and angulation of the mandible pre and post treatment with extraction (r<0.5). Conclusion: No significant difference was observed in mandible’s rotation post orthodontic treatment in the position and angulation of the condyle of skeletal Class II patients with extraction treatment. Statistically significant difference was found in patients post orthodontic treatment without extraction and condyle’s position (Co-Tvertical) in skeletal class II.

Latar Belakang: Perawatan maloklusi Klas II skeletal dapat terjadi perubahan dataran oklusal, posisi dan rotasi mandibula yang dapat mempengaruhi dimensi vertikal wajah dan profil wajah. Perubahan rotasi mandibula yang terjadi pada perawatan maloklusi Kelas II skeletal sedikit banyak nya mempengaruhi posisi dan angulasi kondilus mandibula. Namun perubahan rotasi mandibula yang terjadi setelah perawatan ortodonti masih diperdebatkan. Tujuan: Untuk mengetahui hubungan perubahan rotasi mandibula terhadap posisi dan angulasi kondilus pada mlaoklusi Klas II skeletal setelah perawatan ortodonti. Metode: Penelitian ini merupakan penelitian cross sectional dengan sampel radiografi sefalometri lateral sebelum dan sesudah perawatan dari 30 pasien yang dirawat dengan ektraksi maupun non ekstraksi. Perubahan rotasi mandibula diukur dari perubahan sudut MP-SN sedangkan perubahan posisi diukur dari jarak Co-TC, jarak Co-Tvertikal dan sudut kondilus diukur dari sudut Co-T-T vertikal. Hasil: Tidak terdapat perubahan yang signfikan pada rotasi mandibula sebelum dan sesudah perawatan baik pada ekstraksi maupun non ekstraksi (p>0.05). Terdapat perubahan yang signifikan pada angulasi mandibula sebelum dan sesudah perawatan dengan ekstraksi (p<0.05). Terdapat korelasiyang kuat antara perubahan rotasi mandibula terhadap posisi kondilus (Co-Tvertikal) pada non ekstraksi (r>0.5). Tidak terdapat korelasi yaang kuat antara perubahan rotasi mandibula terhadap posisi dan angulasi kondilus sebelum dan sesudah perawatan dengan ekstraksi (r<0.5). Kesimpulan: Tidak ada hubungan yang signifikan pada perubahan rotasi mandibula yang terjadi setelah perawatan ortodonti terhadap posisi dan angulasi kodilus pada malokui Kelas II skeletal dengan ekstraksi. Perubahan rotasi mandibula yang terjadi setelah perawatan non ekstraksi memberikan hubungan yang signifikan terhadap posisi kondilus (Co-Tvertikal) pada maloklusi Kelas II skeletal.
