    Efektifitas Ekstrak Etanol dan Fraksi Etilasetat Biji Pinang (Areca catechu L.) terhadap Perubahan Intensitas Infeksi Cacing, Kadar IL-9, IL-10, IL-18 dan Histopatologi Kolon pada Mencit (Mus musculus) yang Diberi Telur Infektif Trichuris Muris Peroral

    Abstract
    Latar belakang : Penelitian yang dilakukan pada infeksi cacing T. muris telah menunjukkan pada kita bagaimana sistem imun menginduksi ekspulsi parasit, sehingga dapat diaplikasikan lebih luas pada perkembangan terapi terbaru infeksi STH. Pada saat terinfeksi, sitokin interleukin-18 (IL-18) yang berperan terjadinya ganggunan saluran cerna kronik dan interleukin-10 (IL-10) berperan pada pemeliharaan fungsi pertahanan kolon (colon barrier). Selanjutnya, penelitian pengembangan bahan-bahan alam dan tradisional untuk obat alternatif antihelmintik telah dilakukan, dan salah satu tanaman obat alternatif yang dipilih adalah biji pinang Areca catechu L. (A. catechu L.). Tujuan penelitian : melihat efektivitas dari ekstrak etanol dan fraksi etilasetat terhadap perubahan intensitas infeksi cacing, kadar IL-9, IL-10, IL-18 Dan Histopatologi Kolon Pada Mencit (Mus musculus) Yang Diberi Telur Infektif Trichuris Muris Peroral. Metode : Penelitian ini merupakan penelitian eksperimental dengan rancangan penelitian post test only control group design, menggunakan mencit putih jantan (Mus musculus L.) yang dirandomisasi dalam 10 kelompok yakni kelompok kontrol negatif, kelompok kontrol positif, kelompok yang mendapatkan ekstrak etanol biji pinang 100 mg/kgBB dan 150 mg/kgBB, fraksi etil asetat biji pinang dengan dosis 100 mg/kgBB, dan 150 mg/kgBB maupun albendazol 1mg/20g. Hasil penelitian : menunjukkan bahwa ekstrak etanol biji pinang 150mg/KgBB memiliki efektifitas sebagai antihelmintik lebih baik dibandingkan dengan ekstrak etanol biji pinang 100mg/kgBB, fraksi etilasetat biji pinang 100 mg/KgBB dan 150mg/KgBB dan albendazol 1mg/20g. Ekstrak etanol biji pinang dapat meningkatkan kadar IL-10 dan IL-18 serum darah dan kadar IHK IL-18 pada jaringan kolon. Kesimpulan : Ekstrak etanol biji pinang dapat meningkatkan IL-10 dan IL-18 serum darah yang berfungsi sebagai pertahanan dan mencegah gangguan saluran cerna menjadi kronis karena kandungan alkaloid dan flavonoid serta memiliki efektifitas sebagai antiinflamasi dalam mencegah terjadinya infeksi kronis pada usus besar yang disebabkan infeksi T. muris,
     
    Background: Previous research on the infection of T. muris has shown us the mechanism of the immune system in inducing parasite expulsion, which further is widely applicable to the development of new therapeutic options for the infection of soil-transmitted helminth (STH). During the infection, cytokine interleukin-18 (IL-18) is known to play a role in the occurrence of chronic digestion disorder, while interleukin-10 (IL-10) plays a role in the maintenance of colon barrier function. Research in the development of natural or traditional material for alternative anthelminthic medicine is currently being developed extensively, with one potential plant chose; pinang nuts (Areca catechu L.) Objectives: This research aims to analyze the effectiveness of ethanol extract and the ethyl-acetate extract of pinang nuts (Areca catechu L.) to the immunological reaction that happened in mice (Mus musculus) perorally infected with infective Trichuris muris. It includes the alteration of helminth’s infection intensity, the level of IL-9, IL-10, IL-18, as well as changes in the mice’s colon histopathology. Methods: This study is an experimental research using a post-test only control group design, with male albino mice (Mus musculus L) being randomized into ten groups. They consisted of negative control, positive control, groups treated with pinang nut ethanol extract of 100 mg/kg b.w., and 150 mg/kg b.w., groups treated with pinang nut ethyl-acetate fraction of pinang nuts of 100 mg/kg b.w., and 150 mg/kg b.w., as well as group treated with albendazole of 1 mg/20 g. Results: The result showed that the pinang nut ethanol extract of 150 mg/kg b.w. show more effective anthelminthic action compared to ethanol extract of 100 mg/kg b.w., the ethyl-acetate fraction of 100 mg/kg b.w., and 150 mg/kg b.w., and better compared to albendazole of 1 mg/20 g treatment. The ethanol extract of pinang nuts could increase the level of IL-10 and IL-18 in blood serum, as well as the IL-18 level in immunohistochemistry (IHC) analysis on colon tissue. Conclusion: The ethanol extract of pinang nuts could increase the level of IL-10 and IL-18 in blood serum, with alkaloid and flavonoid content serve as protection for digestion to prevent the development of the chronic digestive disorder. The ethanol extract also has an anti-inflammation effect to avoid the occurrence of colitis or chronic infection in the colon due to T. muris infection.
