    Hubungan Kadar HbA1c Terhadap Perubahan Optic Nerve Head dan Defek Lapang Pandangan pada Penderita POAG dengan Riwayat DM type 2

    Abstract
    Objective: The aim of this study was to determine the relationship between HbA1c levels and changes in ONH and visual field defects in POAG patients with a history of type 2 diabetic. Methods: This was an observational analytic study with consecutive sampling conducted at the Medan Baru Eye Hospital. Cases was taken from patients POAG with a history of type 2 diabetic until inclusion criteria were reached and then the relationship of each variable was based on age, sex and duration of type 2 diabetic. Demographic, anthropometric and medical characteristics data were collected from 66 subjects, with anamnesis and ophthalmology examination. Results: The cases were taken from 66 patients POAG with history of type 2 diabetic and have been analized by SPSS 26 software. The data obtained were normally distributed using the Kolmogorov-Smirnov test and by the Chi-square test with Pearson Corelation and Fisher’s Exact Test obtained a significant relationship between HbA1c levels and CDR changes in ONH and visual field defects based on the duration of suffering from DM. However, there was no significant relationship based on age and sex. The results was showed as the following: The relationship between HbA1c levels based on CDR (mean deviation : 0,55 ± 0,15; p = 0,012), MD ( mean deviation = -10,31 ± 5,927; p = 0,019), PSD ( mean deviation = 9,39 ± 7,28, p = 0,027). The relationship between HbA1c levels, CDR, MD, PSD based on age ( mean deviation 55,61 ± 7,20; p=(0,082, 0,051, 0,046, 0,027), based on gender (p = 0,103, 0,158, 0,138, 0,071, 0,173, 0,087), based on the duration of type 2 DM ( mean deviation = 7,65 ± 4,29; p = 0,012, 0,020, 0,027). Conclusion: This study found that the elevated HbA1c levels in POAG patients with a history of type 2 diabetic will affect the optic nerve damage characterized by CDR changes in ONH and visual field defects based on the duration of type 2 diabetic. However, in this study, optic nerve damage and HbA1c levels was not correlated significantly based on age and gender.
     
    Tujuan: Tujuan dari penelitian ini adalah untuk mengetahui hubungan kadar HbA1c terhadap perubahan ONH dan defek lapang pandangan pada pasien POAG dengan riwayat DM tipe 2. Metode: Merupakan penelitian analitik observasional dengan consecutive sampling yang dilakukan di Rumah Sakit Khusus Mata Medan Baru. Kasus yang diambil selama periode penelitian ialah penderita POAG dengan riwayat DM tipe 2 sampai tercapainya kriteria inklusi dan kemudian hubungan setiap variabel didasarkan atas usia, jenis kelamin dan lamanya menderita DM tipe 2. Data demografi, antropometrik serta karakteristik medis dikumpulkan dari 66 subjek, dengan anamnesis dan pemeriksaan oftalmologi. Hasil: Kasus yang diambil adalah 66 pasien POAG dengan riwayat DM tipe 2 dan dianalisis dengan menggunakan piranti lunak SPSS 26. Data yang didapatkan terdistribusi normal dengan uji Kolmogorov-Smirnov dan dengan dengan menggunakan uji Chi-square dengan Pearson Corelation dan Fisher’s Exact Test didapat hubungan yang signifikan antara hubungan kadar HbA1c terhadap perubahan CDR pada ONH dan defek lapang pandangan berdasarkan lamanya menderita DM. Namun tidak dijumpai hubungan yang signifikan berdasarkan usia dan jenis kelamin. Hasil penelitian menunjukkan sebagai berikut: Hubungan Kadar HbA1c berdasarkan CDR (mean deviasi : 0,55 ± 0,15; p = 0,012), MD ( mean deviasi = -10,31 ± 5,927; p = 0,019), PSD ( mean deviasi = 9,39 ± 7,28, p = 0,027). Hubungan Kadar HbA1c, CDR, MD, PSD berdasarkan usia ( mean deviasi 55,61 ± 7,20; p=(0,082, 0,051, 0,046, 0,027), berdasarkan jenis kelamin (p = 0,103, 0,158, 0,138, 0,071, 0,173, 0,087), berdasarkan lamanya DM tipe 2 ( mean deviasi = 7,65 ± 4,29; p = 0,012, 0,020, 0,027). Kesimpulan: Pada penelitian ini ditemukan bahwa kadar HbA1c yang meningkat pada pasien POAG dengan riwayat DM tipe 2 akan mempengaruhi kerusakan saraf optik yang ditandai dengan perubahan CDR pada ONH dan defek lapang pandangan yang didasarkan atas lamanya pasien menderita DM tipe 2. Namun pada penelitian ini, kerusakan saraf optik tidak signifikan terhadap usia dan jenis kelamin.
