    Karakteristik Genotip dan Alel Enzim Microsomal Epoxide Hydrolase (EPHX1) His139Arg pada Penderita Kanker Paru di RSUP H. Adam Malik Medan

    Abstract
    Background : Microsomal epoxide hydrolase 1 (EPHX1) plays an important role in both activation and the detoxification of polycyclic aromatic hydrocarbons and aromatic amines. Polymorphisms at exon 4 of the EPHX1 gene have been reported to be associated with variation in EPHX1 activity. Many studies have investigated the association between EPHX1 His139Arg polymorphisms and the lung cancer risk, but the impact of EPHX1 His139Arg polymorphisms on lung cancer risk is not clear owing to the apparent inconsistence among those studies. This study aims to determine the frequency distribution of genotypes and enzymes allel Microsomal Epoxide Hydrolase (EPHX1) His139Arg in patients with lung cancer in H. Adam Malik General Hospital Medan. Material and Method : This is a descriptive study, began from January 2017 up to April 2017. Population of this study is 42 lung cancer patients in Haji Adam Malik General Hospital, Medan – Indonesia. All samples are men and smoker or ex-smoker. We investigated the EPHX1 His139Arg gene polymorphisms using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) in Terpadu Laboratorium, Faculty Medicine University of Sumatera Utara. Result : Of all 42 subjects, wild-type variant His139His were 34 (81%), heterozygote variant His139Arg were 8 (19%), there was no homozygote variant Arg139Arg identified. Characteristics of lung cancer patients were found at most aged 40-59 years (54.76%), with a heavy Brinkman Index of 36 people (85.71%) and the most widely used type of cigarette was kretek of 23 people (54.80% ). The most common type of lung cancer cell histopathology is the type of adenocarcinoma in 35 cases (83.30%). Conclusion : The His139His EPHX1 genotype variant is the most common polymorphism in lung cancer patients in RSUP. H Adam Malik Medan.
     
    Latar Belakang : Microsomal epoxide hydrolase 1 (EPHX1) memainkan peran penting dalam aktivasi dan detoksifikasi hidrokarbon aromatik polisiklik dan amina aromatik. Polimorfisme pada ekson 4 gen EPHX1 telah dilaporkan terkait dengan variasi aktivitas EPHX1. Banyak penelitian telah menyelidiki hubungan antara polimorfisme EPHX1 His139Arg dan risiko kanker paru-paru, namun dampak polimorfisme EPHX1 His139Arg pada risiko kanker paru-paru tidak jelas karena ketidakkonsistenan di antara penelitian tersebut. Penelitian ini bertujuan untuk mengetahui distribusi frekuensi genotip dan alel enzim Microsomal Epoxide Hydrolase (EPHX1) His139Arg pada penderita kanker paru di RSUP H. Adam Malik Medan. Bahan dan Metode : Ini adalah studi deskriptif dimulai dari bulan Januari 2017 sampai April 2017. Populasi penelitian ini adalah 42 pasien kanker paru-paru di Rumah Sakit Umum Haji Adam Malik, Medan - Indonesia. Semua sampel adalah pria dan perokok atau mantan perokok. Kami menyelidiki polimorfisme gen EPHX1 His139Arg menggunakan polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) di Laboratorium Terpadu, Fakultas Kedokteran Universitas Sumatera Utara. Hasil : Dari ke 42 subyek, varian wild-type His139His adalah 34 (81%), varian heterozigot His139Arg adalah 8 (19%), tidak ada varian homozigot Arg139Arg diidentifikasi. Karakteristik pasien kanker paru dijumpai paling banyak berumur 40-59 tahun (54,76%), dengan Indeks Brinkman berat yaitu sebanyak 36 orang (85,71%) dan jenis rokok yang paling banyak digunakan adalah kretek sebanyak 23 orang (54,80%). Jenis histopatologi sel kanker paru paling banyak ditemui adalah jenis adenokarsinoma yaitu 35 kasus (83,30%). Kesimpulan : Varian genotip EPHX1 His139His merupakan polimorfisme paling umum pada pasien kanker paru di RSUP. H Adam Malik Medan.
