dc.description.abstractIntroduction: Chronic itch is still public concern because of high incidence and effect on decreasing quality of life. Pruritus, which is the main symptom of atopic dermatitis (AD), involves a variety of molecules in its pathogenesis and other dermatitis. The newly known pruritogen is interleukin (IL) 31. The heterogeneity of causes of itching and various therapeutic approaches remains a challenge for clinicians. Objective: This study aims to determine the relationship between serum IL-31 levels and pruritus scale in adult AD patients and adult non-AD patients. Methods: This study was an analytic study using cross-sectional method. Fortyfour samples were taken from patients aged ≥18 years, 22 were diagnosed with AD and the remaining 22 included allergic contact dermatitis, prurigo nodularis, urticaria, uremic pruritus, psoriasis and chronic lichen simplex. All patients were assessed on a 5D itch scale, received informed consent at the Dermatovenereology Polyclinic of general hospital HAM Medan followed by examination of serum IL- 31 levels. Patients who received steroid, antihistamine and immunomodulatory therapy in the last 7 days or needed long-term therapy, were not fully conscious or taking opioids were excluded. The collected data were analyzed by Spearman’s test. Results: There was no significant relationship between serum IL-31 levels and pruritus scale in adult AD patients, but there was significant relationship in adult non-AD patients. Discussion: IL-31 can inhibit eosinophil apoptosis and increase inflammatory response by stimulating the secretion of proinflammatory cytokines and chemokines resulting in infiltration of inflammatory cells into skin lesions. AD is a multifactorial disease and there were other pruritogens that play role in pathogenesis. Conclusion: IL31 affects the pruritus scale in non-AD disease. Further research is needed to analyze the relationship between other pruritogens and pruritus scale in AD by comparing AD lesions and nonlesions.en_US
dc.description.abstractLatar Belakang: Gatal kronik masih menjadi perhatian masyarakat karena angka kejadian yang tinggi dan berefek terhadap penurunan kualitas hidup. Pruritus yang merupakan gejala utama dermatitis atopik (DA), melibatkan berbagai macam molekul dalam patogenesisnya maupun dermatitis lainnya. Pruritogen yang baru diketahui adalah interleukin (IL) 31. Heterogenitas penyebab gatal dan berbagai pendekatan terapi masih menjadi tantangan klinisi. Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan antara kadar IL-31 serum dengan skala pruritus pada pasien DA dewasa dan pada pasien non-DA dewasa. Metode: Penelitian ini merupakan studi analitik dengan metode potong lintang. Empat puluh empat sampel diambil dari pasien yang berusia ≥18 tahun, 22 didiagnosis DA dan 22 sisanya mencakup dermatitis kontak alergi, prurigo nodularis, urtikaria, pruritus uremik, psoriasis dan liken simpleks kronis. Seluruh pasien dilakukan penilaian skala gatal 5D, mendapat Informed consent di Poliklinik Dermatovenereologi RSUP HAM Medan kemudian dilakukan pemeriksaan IL-31 serum. Pasien yang mendapatkan terapi steroid, antihistamin serta immunomodulator dalam 7 hari terakhir atau perlu mendapatkan terapi jangka panjang, tidak dalam kesadaran penuh atau mengkonsumsi zat opioid dieksklusikan. Data yang terkumpul dianalisis dengan uji Spearman. Hasil: Tidak terdapat hubungan signifikan antara kadar IL-31 serum dengan skala pruritus pada pasien DA dewasa, namun terdapat hubungan yang signifikan pada pasien non-DA dewasa. Diskusi: IL-31 dapat menghambat apoptosis eosinofil dan meningkatkan respon inflamasi melalui stimulasi sekresi sitokin dan kemokin proinflamasi sehingga terjadi infiltrasi sel radang ke lesi kulit. DA merupakan penyakit multifaktorial dan terdapat pruritogen lainnya yang berperan dalam patogenesis. Kesimpulan: IL-31 mempengaruhi skala pruritus pada penyakit non-DA. Diperlukan penelitian lanjut untuk menganalisis hubungan pruritogen lainnya dengan skala pruritus pada DA dengan membandingkan DA lesi dan nonlesi.en_US
