    Asuhan Kefarmasian sebagai Strategi Manajemen Terapi pada Pasien Hiperlipidemia di Apotek

    Abstract
    Hiperlipidemia merupakan penyakit degeneratif dimana meningkatnya konsentrasi kolesterol total, low-density lipoprotein (LDL), trigliserida serta penurunan konsentrasi high-density lipoprotein (HDL) dalam darah secara abnormal. Apoteker sebagai tenaga kesehatan mempunyai peran dalam meningkatkan kualitas hidup pasien hiperlipidemia. Tujuan penelitian ini adalah mengevaluasi dampak asuhan kefarmasian sebagai strategi manajemen terapi pada pasien hiperlipidemia di apotek. Penelitian ini menggunakan metode cohort study terhadap 45 pasien hiperlipidemia di Apotek Gratia Farma 3 periode Juli-Oktober 2020. Kejadian Drug Related Problems (DRPs) diidentifikasi menggunakan instrument Pharmaceutical Care Network Europe (PCNE) V9.00, tingkat kepatuhan dinilai dengan kuesioner Morisky Medication Adherence Scale-8 (MMAS-8), outcome terapi dengan mengukur kadar profil lipid, dan kualitas hidup pasien diukur dengan menggunakan European Quality of Life-5 Dimension 3 Level (EQ-5D-3L). Data sebelum (pretest) dan sesudah (post-test) intervensi dianalisis dengan menggunakan uji wilcoxon pada program SPSS V.22. Penerapan asuhan kefarmasian menurunkan jumlah kejadian DRPs dari 2,71 ± 0,59 (pre-test) menjadi 0,84 ± 1,19 (post-test), meningkatkan kepatuhan pasien hiperlipidemia dari 5,09 ± 1,10 (pre-test) menjadi 6,73 ± 0,58 (post-test), memperbaiki profil lipid pasien hiperlipidemia dari pre-test (kolesterol total = 235,96 ± 22,89; LDL = 149,31 ± 20,57; HDL = 42,89 ± 6,75; trigliserida = 233,33 ± 73,37) menjadi post-test (kolesterol total = 204,80 ± 8,97; LDL = 108,13 ± 7,35; HDL = 65,80 ± 4,58; trigliserida = 154,51 ± 14,17), dan meningkatkan kualitas hidup pasien hiperlipidemia dari 0,656 ± 0,2 (pre-test) menjadi 0,891 ± 0,11 (posttest). Penerapan asuhan kefarmasian sebagai strategi manajemen terapi menurunkan jumlah kejadian DRPs, meningkatkan kepatuhan, outcome terapi, dan kualitas hidup pada pasien hiperlipidemia di apotek.
     
    Hyperlipidemia is a degenerative disease with abnormally elevated levels of total cholesterol, low-density lipoprotein (LDL), triglycerides and decreased highdensity lipoprotein (HDL). Pharmacists as health care provider have a role in improving the quality of life of hyperlipidemic patients. The study evaluated the impact of pharmaceutical care as a therapeutic management strategy in hyperlipidemic patients in pharmacy. The cohort study involved 45 hyperlipidemic patients at Gratia Farma 3 Pharmacy in July-October 2020. Drug Related Problems (DRPs) was identified by Pharmaceutical Care Network Europe (PCNE) V9.00 instrument, the adherence was assessed by the Morisky Medication Adherence Scale-8 (MMAS-8) questionnaire, therapeutic outcome by measuring of lipid profile, and the quality of life with the European Quality of Life-5 Dimension 3 Level (EQ5D3L) questionaire. Data before (pre-test) and after (post-test) the intervention were analyzed using the wilcoxon test in SPSS program V.22. The pharmaceutical care implementation decreased the occurence of DRPs from 2.71 ± 0.59 (pre-test) to 0.84 ± 1.19 (post-test), increased the adherence of hyperlipidemic patients from 5.09 ± 1.10 (pre-test) to 6.73 ± 0.58 (post-test), improving the lipid profile of hyperlipidemic patients from pre-test (total cholesterol = 235.96 ± 22.89; LDL = 149.31 ± 20.57; HDL = 42.89 ± 6.75; triglycerides = 233.33 ± 73.37) to post-test (total cholesterol = 204.80 ± 8.97; LDL = 108.13 ± 7.35; HDL = 65.80 ± 4.58; triglycerides = 154.51 ± 14.17), and improved the quality of life from 0.656 ± 0.2 (pre-test) to 0.891 ± 0.11 (posttest). The pharmaceutical care as a therapeutic management strategy decreased the occurence of DRPs, improved therapeutic outcomes, adherence and quality of life in hyperlipidemic patients in pharmacy.
