    Perbandingan Efektivitas Albendazole 5 dan 7 Hari pada Infeksi Trichuris Trichiuria

    Abstract
    ABSTRACT Background. The worm infection is almost occurred in all of people at all age, but the high prevelance especially age school children. Trichuris trichiura is one of the most common soil-transmitted helminths that infected school age children. Single dose albendazole showed wide variation in of cure and egg reduction rate. Some studies found repeated dose of albendazole would increase its effectiveness. Objective. To determine the effectiveness of 400 mg albendazole five consecutive days compare to 400 mg albendazole seven consecutive days against trichiuriasis. Methods. A randomized, open clinical trial was conducted between August and September 2009 among elementary school children at Jaring Halus, North Sumatera Province. Stool samples were collected before treatment and on day 7,14, 21, and 28 after treatment using the Kato Katz method. Group I received 400 mg albendazole five consecutive days and group II received 400 mg albendazole seven consecutive days. Cure rate and egg reduction rate was compared using Chi-square and Student’s t test respectively. Result. One hundred and twenty one subjects enrolled (group I n= 61, group II n=60). Treatments for seven days resulted in significantly higher cure rate compared with five days, at first week 86,7% for seven days compared to 39,3% (P=0.001) and second week 88,3% for seven days compared to 68,9% (P=0.017). Egg reduction rates was also statistically better in group I 6.6% (SD 11.30) compared to group II 20.3% (SD 23.77). Conclusion. Five and seven days albendazole are equally effective against trichuriasis. Albendazole for seven consecutive days has a higher effectiveness in egg reduction rate compared than five consecutive days. Keywords: albendazole, trichuriasis, single dose, repeated doses
     
    ABSTRAK Latar belakang. Infeksi cacing mengenai semua orang di segala usia, tetapi prevalensi tertinggi terutama terjadi pada anak-anak usia sekolah. Trichuris trichiura adalah salah satu cacing dari soil-transmitted helminths yang banyak menginfeksi anak sekolah. Albendazole menunjukkan beberapa variasi yang luas dalam menyembuhkan dan menurunkan jumlah telur. Beberapa penelitian menunjukkan pengulangan dosis dari albendazole akan meningkatkan keefektivannya. Tujuan. Untuk membandingkan efektivitas pemberian albendazole 400 mg 5 hari berturut dan tujuh hari berturut terhadap trichuriasis. Metode. Uji klinis terbuka terhadap anak-anak usia sekolah dasar dilakukan selama Agustus dan September 2009, di Kabupaten Langkat, Propinsi Sumatera Utara. Sampel tinja diperiksa menggunakan metode Kato-Katz sebelum dan sesudah hari ke 7, 14, 21, dan 28 pemberian obat. Kelompok I mendapatkan terapi albendazole 400 mg selama 5 hari berturut dan kelompok II mendapatkan albendazole 400 mg selama 7 hari berturut. Uji Chi-square dan Student’s t test digunakan untuk membandingkan angka kesembuhan dan penurunan telur diantara kedua kelompok. Hasil. Penelitian ini mengikutsertakan 121 subjek (kelompok I n= 61, kelompok II n= 60). Kesembuhan pada kelompok II menunjukkan angka yang lebih tinggi 86.7% dibandingkan pada kelompok I yaitu 39.3% (P=0.001) pada minggu pertama dan kedua yaitu 88.3% dan 68.9% (P=0.017). Penurunan telur pada kelompok II juga menunjukkan angka yang lebih tinggi yaitu 20.3% dibandingkan kelompok I yaitu 6.6% (P=0.001). Kesimpulan. Albendazole 400 mg lima hari berturut dan tujuh hari menunjukkan efektivitas yang sama tinggi terhadap angka infeksi T. trichiura. Pemberian regimen albendazole 400 mg tujuh hari berturut lebih efektif daripada albendazole 400 mg lima hari dalam penurunan jumlah telur.
