    Korelasi antara Jumlah Asupan Kalori dan Protein dengan Imbang Nitrogen pada Anak Sakit Kritis di Unit Rawatan Intensif Anak

    Abstract
    Background Critically ill child has limited metabolic ability to deal with stress, so without the aid of additional energy both protein and non-protein child will easily fall into a catabolic state. One way to measure the status of protein metabolism is the nitrogen balance. Objective To assess the correlation between caloric and protein intake with nitrogen balance in critically ill children Methods Cross-sectional study conducted from April to June 2016 in the Pediatric Intensive Care Unit Haji Adam Malik Hospital. Nitrogen balance is calculated as the difference between protein intake and urine levels of N Urea. Spearman and Pearson test was performed to assess the correlation between the caloric and protein intake with nitrogen balance. Results Of the 37 subjects, 35 experienced a negative nitrogen balance with a mean of -2.24 g (SB 1.40). The mean of protein intake was 1:47 g / kg / day (SB 1:02). Weak correlation obtained between caloric intake with nitrogen balance (r = 0.319, p = 0.057), whereas strong and significant correlation between protein intake with nitrogen balance (r = 0610, p = <0.001). Conclusions Nitrogen balance significantly correlated with protein intake, but weakly correlated with caloric intake.
     
    Latar belakang Anak sakit kritis memiliki kemampuan metabolisme terbatas dalam menghadapi stres, sehingga tanpa bantuan tambahan energi baik protein maupun non protein anak akan mudah jatuh kedalam keadaan katabolik. Salah satu cara mengetahui status metabolisme protein adalah imbang nitrogen. Tujuan Menilai korelasi antara jumlah supan kalori dan protein dengan imbang nitrogen pada anak sakit kritis. Metode Dilakukan studi cross-sectional pada April sampai Juni 2016 di Unit Perawatan Intensif Anak Rumah Sakit Haji Adam Malik Medan. Imbang nitrogen dihitung berdasarkan rumus selisih asupan protein dan kadar N Urea Urin. Uji Spearman dan Pearson dilakukan untuk menilai korelasi antara jumlah asupan kalori dan protein dengan imbang nitrogen Hasil Dari 37 subyek, 35 mengalami imbang nitrogen negatif dengan rerata -2.24 gr (SB 1,40). Didapatkan rata rata kadar pemberian protein 1.47 gr/kg/hari (SB 1.02). Didapatkan korelasi lemah antara asupan kalori dengan imbang nitrogen (r = 0.319, p = 0,057), sebaliknya korelasi kuat dan signifikan antara asupan protein dengan imbang nitrogen ( r = 0.610, p = <0,001) Kesimpulan Imbang nitrogen berkorelasi kuat dengan asupan protein, tetapi berkorelasi lemah dengan asupan kalori.
