Perbedaan Profil Lipid Serum Sebelum dan Sesudah Pengobatan Olanzapin pada Pasien Skizofrenik

Abstract

Background: The Increased use of atypical antipsychotics, such as olanzapine, for schizophrenia treatment and serious psychotic illnesses has increased the possibility of side effects of lipid abnormalities. Objective : to assess changes in serum lipid profile in the use of atypical antipsychotic olanzapine before and after 4 weeks of treatment. Methods: This study is an experimental study of one group pretest posttest design, open trial, the provision of olanzapine in acute phase 55 schizophrenic subjects for 4 weeks before and after treatment to see the lipid profile (total cholesterol, triglycerides, HDL , LDL) Results: The results of baseline total cholesterol levels mean 159.2 ± 20.4. After treatment the mean total cholesterol obtained 168.8 ± 21.6 and found the value of p = 0,0001. Mean baseline triglyceride level 95.8 ± 19 and after treatment the mean triglyceride 139.1 ± 25.1 p = 0.0001 found HDL . Mean baseline level with 46.3 ± 10.1 and mean after treatment found 49.7 ± 9.1 HDL and found the value of p = 0.0001. Mean baseline LDL levels to 104.3 ± 18.5 and after treatment found mean LDL 103.5 ± 16.6 and found the value of p = 0.675. Conclusion: Olanzapine might increase the levels of total cholesterol, triglyceride, and HDL whilst the level of LDL might decrease in patients with schizophrenia for 4 weeks who use it.

Latar belakang: Peningkatan pemakaian antipsikotik atipikal termasuk olanzapin untuk pengobatan skizofrenia dan penyakit psikotik yang serius, telah meningkatkan kemungkinan terjadi efek samping abnormalitas lipid dalam darah. Studi ini bertujuan untuk menilai perubahan profil lipid pada pemakaian antipsikotik atipikal olanzapin sebelum dan sesudah 4 minggu pengobatan. Metode: studi ini adalah studi experimental one group pretest-posttest design, open trial, tidak dirandomisasi, pemberian olanzapin pada 55 subjek skizofrenik fase akut selama 4 minggu dimana sebelum dan sesudah pengobatan dilakukan pemeriksaan laboratorium darah untuk melihat profil lipid (kolesterol total, trigliserida, HDL, LDL) Hasil: kadar kolesterol total baseline dengan rerata 159,2±20,4. Setelah pengobatan didapat kolesterol total rerata 168,8±21,6 dan dijumpai nilai p=0,0001.Kadar trigliserida baseline rerata 95,8±19 dan setelah pengobatan rerata trigliserida 139,1±25,1 dijumpai nilai p=0,0001.kadar HDL baseline dengan rerata 46.3±10.1 dan setelah pengobatan dijumpai rerata HDL 49,7±9,1 dan dijumpai nilai p=0,0001. Kadar LDL baseline dengan rerata 104,3±18,5 dan setelah pengobatan dijumpai rerata LDL 103,5±16,6 dan dijumpai nilai p=0,675. Kesimpulan: Pada pengobatan Olanzapin selama 4 minggu pada pasien skizofrenik dijumpai peningkatan kadar kolesterol total, kadar trigliserida dan kadar HDL yang bermakna, dan dijumpai penurunan LDL tetapi tidak ditemukan perbedaan bermakna.
