Introduction :Identification of skeletal maturation in growning subject is very important forthe orthodontist to determine the ideal diagnose and treatment planning. The use of middle phalanx-3 (MP3) radiography, based on Rajagopal and Kansal modification method in determining the level of skeletal maturation, has significant correlations with mandibular and maxillary growth. The increase in alkaline phosphatase (ALP) level is one of the indications of the incidence of bone remineralization. Aim of study: The objective of this study was to assess salivary alkaline phosphataselevels in growing female subjects in relation to stages of MP3 maturation. Materials and methods :The sample of this cross sectional study consisted of 56 subjects, in the age ranged from 8-15 years. The subjects were divided into three groups according to their growth phases, that were pre-pubertal (MP3-F and MP3-FG), pubertal (MP3-G and MP3-H), and post-pubertal (MP3-HI and MP3-I). Growth phase was assessed through the MP-3 maturation on periapical radiograph using the method described by Rajagopal and Kansal. Passive drool method was used in taking 1-2 ml of saliva. Level of salivary alkaline phosphatase was determined spectrophotometrically. Data were analyzed by using Oneway ANOVA and Least Significant Difference test.Result : Statistical test revealed the present of significant difference between the alkaline phosphatase in the MP-3 maturation (p=0.0001) and the highest ALP content in the pubertal phase of 224.22 ± 136.76 (IU/L), in the pre-pubertal phase of 189.06 ± 71.76 (IU/L), and in the lowest post-pubertal of 73.81 ± 26.07 (IU/L). Conclusion : Salivary alkaline phosphatase can be used as a non invasive biomarker for the identification of the pubertal growth phase to determine orthodonic treatment timing.en_US
