    Pemantauan Efektivitas Terapi Gentamisin Dosis Berganda Bolus Intravenus terhadap Infeksi pada Penyakit Paru Obstruktif Kronis

    Abstract
    It has been carried out a study on the effect of therapeutic drug monitoring of gentamisin population dose 80 mg toward chronic obstructive pulmonary disease at Pulmonology and Respiratory department RSUP.H Adam Malik Medan,. The gentamycin is given bolus intravenously every 12 hours toward 3 sufferer who meet inclusion criteria then the concentration of gentamycin in the blood is done by using aliquot serum taken on 30 minutes after the first dose in steady state, and called as maximum concentration (Cmax). The second aliquot is taken on 6 hours after injection of the first dose to find out clinic manifestation, and the third aliquot is taken 5 minutes before injection of the second dose called as minimum consentration (Cmin). Before and during gentamycin therapeutic, the sufferer undergo laboratory examination covering complete blood, kidney faal (BUN and serum creatinin) and microba resistancy culture test. Based on the examination of gentamycin concentration in the blood by using Chemistry Autoanalyzer COBAS INTEGRA 400 Roche, it is found the result that MU, the first sufferer has Cmax = 4.51 mcg/ml; C6hours= 3.08 mcg/ml; Cmin= 1.34 mcg/ml, SU the second sufferer has Cmax= 4.13 mcg/ml; C6hours= 1.42 mcg/ml, Cmin= 0.52 mcg/ml, MR, the third sufferer had Cmax= 5.22 mcg/ml; C6hours= 0.94 mcg/ml, Cmin= 0.41 mcg/ml. It is calculated on individual dose by steady state formulation had shown and the result that the dose need for MU is 140 mg, SU is 150 mg and MR is 120 mg to get Cmax meeting the therapeutic range and effective toward the infection recovery. The result the of study shows that therapy of gentamycin 80 mg every 12 hours without considering of body weight and kidney function cause big variability toward the achievement of therapeutic consentration.
     
    Telah dilakukan penelitian terhadap efektifitas terapi gentamisin dosis populasi 80 mg pada penderita Penyakit Paru Obstruktif Kronis di departemen Ilmu Penyakit Paru dan Saluran Pernafasan RSUP. H. Adam Malik Medan. Gentamisin diberikan secara intravenus bolus tiap 12 jam pada 3 orang penderita yang memenuhi kriteria inklusi kemudian diamati kadarnya di dalam darah penderita. Pengamatan terhadap kadar gentamisin dalam darah dilakukan dengan menggunakan cuplikan serum yang diambil pada 30 menit setelah dosis ke-1 dalam keadaan tunak, dan dinyatakan sebagai konsentrasi maksimum (Cmax). Cuplikan kedua diambil pada 6 jam setelah injeksi dosis ke-1 untuk mengetahui manifestasi klinis, dan cuplikan ketiga diambil 5 menit sebelum injeksi dosis ke-2 yang dinyatakan sebagai konsentrasi minimum (Cmin). Sebelum dan selama terapi gentamisin, penderita menjalani pemeriksaan laboratorium meliputi darah lengkap, faal ginjal (BUN dan kreatinin serum) dan uji kultur resistensi mikroba. Berdasarkan pemeriksaan kadar gentamisin dalam darah menggunakan alat Chemistry Autoanalyzer COBAS INTEGRA 400 Roche diperoleh hasil bahwa penderita MU memiliki Cmax = 4.51 mcg/ml; C6jam= 3.08 mcg/ml; Cmin = 1.34 mcg/ml, penderita SU memiliki Cmax = 4.13 mcg/ml; C6jam= 1.42 mcg/ml, Cmin = 0.52 mcg/ml, penderita MR memiliki Cmax = 5.22 mcg/ml; C6jam= 0.94 mcg/ml, Cmin = 0.41 mcg/ml. Dilakukan perhitungan dosis individual menggunakan rumus Kaedah Keadaan Mantap dan diperoleh hasil bahwa dosis yang dibutuhkan MU adalah sebesar 140 mg, SU sebesar 150 mg, dan MR sebesar 120 mg untuk memperoleh Cmax yang memenuhi range terapi dan efektif terhadap penyembuhan infeksi. Hasil penelitian menunjukkan bahwa pemberian gentamisin dosis 80 mg tiap 12 jam tanpa pertimbangan berat badan dan fungsi ginjal menghasilkan variabilitas yang besar terhadap pencapaian kadar terapi
